    Current situation analysis and model optimization of general category enrollment, general training and major diversion for medical technology specialty

    Objective To analyze the existing problems of the general category enrollment, general training and major diversion for medical technology specialty in order to provide the reference for optimizing and improving the current talent cultivation mode and promote the innovative development of medical technology specialty. Methods In September 2021, a questionnaire survey was used to investigate among 431 students who completed major diversion from grades 2019 and 2020. General linear model was conducted on the scores of compulsory courses in the first academic year and the results of major diversion of grades 2019 and 2020 students to evaluate students' choices of majors. Results The results of 381 valid questionnaires showed that 249(65.4%) students thought that general training could improve their learning enthusiasm and provide help for major diversion, and 217(57.0%) students lacked understanding of general category enrollment policies; 285(74.8%) students were satisfied with the curriculum of general training; 105(27.6%) students felt anxious and depressed after major diversion, and thought that not being assigned to an ideal major would affect their learning enthusiasm. In grade 2019, the average weighted scores of students majoring in medical laboratory technology and medical imaging technology in the first academic year were (82.56±2.94) and (79.15±4.17) respectively, and the scores of students in grade 2020 were (83.30±2.61) and (79.29±2.93) respectively. There are statistical differences between the majors in the two grades (all P<0.001). The proportion of the first choice for the medical laboratory technology specialty of grades 2019 and 2020 are 55.3% (119/215) and 40.7% (88/216) respectively, and the proportion of other majors is less than 30.0%. The trend of major differentiation was found by statistical analyzing the scores and the applications of the students. Conclusions It is necessary to strengthen the publicity of general category enrollment and general training, provide guidance for major selection, focus on the psychological counseling for students after major diversion and explore a new evaluation mechanism for major diversion.

    Practice of and reflection on standardized training for public health doctors in Hubei Province

    In 2018, the National Health Commission of the People's Republic of China organized the pilot work of standardized training for public health doctors in disease prevention and control institutions. Hubei Province, as one of the first pilot provinces, actively explored the training mode and launched the training program in the following year. This paper introduces the standardized training practice of public health doctors in Hubei Province from the aspects of organization and management system construction, teaching staff construction, training mode construction, assessment and evaluation, and put forward some thoughts and suggestions on the current public health physician standardization training, in order to explore the way to establish and improve an effective training system: further establish unified and standardized public health physician standardized training system; strengthen the construction and management of the teaching staff; improve the attractiveness of training programs and explore the education and promotion system cohesion mechanism; establish information management platform to further refine the whole process of training project management.

    Analysis of the status and influencing factors of curriculum-based political and virtuous awareness education among faculty in medical colleges

    Objective To evaluate the current situation of curriculum-based political and virtuous awareness education in medical colleges and identify its influencing factors. Methods A network questionnaire survey was conducted among 311 faculty members in a medical college in Heilongjiang Province. Univariate and multivariate logistic regression analysis was applied to identify and analyze the influencing factors of their curriculum-based political and virtuous awareness education. Results Among 311 faculty members, 197 (63.3%) were able to implement the delivery of this module. The regression analysis results showed that the time for curriculum-based political and virtuous awareness (Wald χ2=12.962,P<0.05,OR<1) and the importance attached by the teaching and research office (Wald χ2=15.471,P<0.05,OR<1) were the protective factors. Conclusions It is common for faculty members in medical colleges to integrate political and virtuous awareness into the medical curriculum. The earlier it has started and the higher the attention was given by the teaching and research office of the teachers, and the more times will be allotted to the delivery.

    The influencing factors and management strategies of early participation in clinical research training for clinical medicine undergraduates

    Objective To explore the influencing factors and management strategies of early participation in clinical research training for clinical medicine undergraduates, and to provide an empirical reference. Methods A questionnaire survey was implemented among 182 undergraduate students in grade 2019 to 2020 majoring in clinical medicine in Peking University Health Science Center from May to June 2022. Multivariate logistic regression model was used to explore factors affecting students' participation in early clinical research training. Results Compared with non-participants, participants had higher grades (OR=6.62, 95%CI: 2.75-15.90, P<0.001), higher proportion of interest in clinical research (OR=4.98, 95%CI: 1.47-16.92, P=0.010) and higher proportion of willingness to obtain research achievements (OR=3.69, 95%CI: 1.18-11.56, P=0.025). The 68 constructive suggestions put forward by students were mainly classified into 22 items (32.4%) of the project management, 13 items (19.1%) of guidance method and 12 items (17.6%) of time arrangement. Conclusions Grade, interest and research achievements are the main factors that influence the early participation of clinical medicine undergraduates in clinical research training. University affiliated hospitals should strengthen publicity and support toward lower grade level, stimulate students' interest, set reasonable goals, improve students' sense of gain and achievement, continue to encourage and support students to participate in clinical research training, and to promote the improvement of students' ability of clinical medicine research.

    Exploration on the construction of mental health safeguard system for pharmaceutical graduate students

    It is one of the top priorities to incorporate the well-being of graduate students into the training system to cultivate the top innovative talents in pharmacy. With the goal of improving psychological crisis recognition and intervention of graduate students majoring in pharmacy, this study selected 71 graduate students majoring in pharmacy in Shanghai Jiaotong University School of Medicine from April 2021 to May 2022 as research participants, conducted a questionnaire survey at important time points in training to identify possible psychological problems, adopted the method of combining individual psychological counseling and group psychological activities to explore the psychological health safeguard system (hereinafter referred to as the safeguard system) for pharmaceutical graduate students, including the postgraduate psychological committee, postgraduate management teachers, and school psychological consultants, and preliminarily evaluated the effect of the safeguard system construction for one year. The results showed that the safeguard system could identify the potential psychological problems of graduate students majoring in pharmacy at an early stage, and a total of 10 graduate students with potential psychological problems were detected. Individual psychological counseling and group psychological activities effectively prevented the occurrence of graduate students' psychological crisis, and alleviated 1 case of peer conflict, 3 cases of uneasy supervisor-postgraduate relations, and 6 cases of academic anxiety. Within one year after the establishment of the safeguard system, there are no graduate students dropping out of school (1 case in the previous year) or requesting tutor transfer (2 cases in the previous year). The construction of the safeguard system and its practical experience can provide empirical reference for the mental health education and crisis intervention identification of university graduate students.

    Application of interactive portrait method in the teaching of anesthesia trainees

    Objective To explore the application of interactive portrait method to improve the teaching of anesthesia trainees. Methods A total of 32 students from the eight-year clinical medicine program of Peking University Health Science Center in 2018 whose internship rotations happened in the Department of Anesthesia of Peking University Third Hospital were enrolled as the research participants, and divided into the experimental group and the control group by random number table method. Eighteen students in the experimental group are trained by interactive portrait method, and 14 students in the control group are trained by traditional method. After the internship, the learning effect is evaluated by knowledge assessment, in-class skill test and questionnaire survey results. Independent sample t-test and χ2 The test were used to compare between the two groups. Results The knowledge test scores of the two groups were (19.43±2.84) and (18.71±1.81), respectively, with no significant difference (t=0.88, P=0.094); The success rate of tracheal intubation and intraspinal anesthesia in the experimental group was 18/18 and 18/18 respectively; while the students in the control group were 12/14 and 14/14 respectively, with no significant difference (both P>0.05). The results of the questionnaire survey showed that the students in the experimental group had higher self-scores than the students in the control group in terms of independently drawing laryngoscopic anatomy [(9.72±0.56) vs. (6.79±2.81)] and completing intraspinal puncture operation [(8.67±1.29) vs. (5.93±2.60)], and the differences were statistically significant (both P<0.05). Conclusions The use of interactive portrait method in anesthesia probation teaching can help students master the basic theory and clinical skills of anesthesia, and is generally recognized by students.

    Application of video feedback teaching method in surgical dressing changing skills training for medical students

    Objective To analyze the effect of video feedback teaching method in the training of surgical dressing changing skills for medical students. Methods The experimental control method was adopted.From April to June 2020 and February to May 2021, 47 five-year clinical medicine students enrolled from 2016 to 2017 who volunteered to participate in the study in the affiliated hospital of a comprehensive university in Shenzhen were recruited as research objects, and were randomly divided into 23 students in the experimental group and 24 students in the control group by random number method. The video feedback teaching method was used for the experimental group whereas the traditional teaching method based on demonstration and practice was used for the control group. The training effect was evaluated by the results of questionnaire survey on dressing changing skills and satisfaction with teaching methods. The final results between the two groups were compared using the t-test or the chi-square. Results There was no significant difference in the scores of the experimental group and the control group before training [(81.61±9.27) vs. (82.79±8.33), P=0.646]. The experimental group's students' performance before and after training was higher than that of the control group [(10.87 ± 7.56) vs. (5.08±8.67)], and the difference was statistically significant (P<0.05). The questionnaire survey results show that the teaching methods of the students in the experimental group help to improve their independent thinking ability [(4.26±0.45) vs. (3.75±0.74)], active classroom atmosphere [(4.13±0.69) vs. (3.46±0.72)], deepened understanding of dressing change knowledge [(4.13±0.63) vs. (3.46±0.88)], improved scores of interest [(4.13±0.69) vs. (3.33±1.01)] and enhancing humanistic care awareness [(4.27±0.63) vs. (3.71±0.86)] than those in the control group, and the differences were statistically significant (all P< 0.001). Conclusions Video feedback teaching method is beneficial to improve clinical medical students' performance in dressing change operation and satisfaction with teaching methods, which has been recognized by students.

    Establishment and evaluation of simplified ABO-OGS oriented teaching for orthodontic brackets bonding

    Objective To reform the traditional teaching method for bonding of straight wire appliance with plaster models, and build up new teaching method that is more reasonable. Methods In May 2019 and May 2021,the experimental course of straight wire appliance bonding in undergraduate Stomatology of Fujian Medical University was selected as the research objects. The 99 students in grade 2017 adopted the simplified ABO-OGS teaching method as the experimental group, while the 101 students in grade 2015 adopted traditional plaster teaching method as the control group. The students and teachers filled in the questionnaire respectively to evaluate the efficacy of the two methods. The scores of the two groups were compared using independent sample t-test. Results In terms of the novelty of experimental teaching methods, the scores of students in the experimental group were higher than those in the control group, and the difference was statistically significant [(5.32 ± 0.80) vs. (4.99 ± 0.90) , P=0.006]. The teacher's evaluation score of the simplified ABO-OGS teaching is higher than that of the traditional teaching, such as mastering the bracket bonding operation [(5.63 ± 0.49) vs. (5.17 ± 0.70)], students' clear understanding of the bracket accuracy [(5.58 ± 0.58) vs. (5.00 ± 0.78)], and can enhance the confidence of the clinical bracket bonding [(5.42 ± 0.78) vs. (5.08 ± 0.78)], and the differences were statistically significant (all P<0.05). Conclusions The simplified ABO-OGS oriented teaching method reasonably improves the teaching instruments for orthodontic brackets bonding, and the teaching method is novel, which helps to establish an objective evaluation system, and is welcomed by students and teachers.

    The exploration and application of presentation-assimilation-discussion combined with case-based learning in pharmacology theory teaching

    Objective To explore the value and promotion feasibility of applying presentation-assimilation-discussion (PAD) class combined with case-based learning (CBL) method in the theory teaching of pharmacology. Methods From August to December 2021, two classes of undergraduate students majoring in clinical medicine in grade 2018 of Jinzhou Medical University were randomly selected as the research subjects and randomly divided into the control group (n=29) and experimental group (n=32). PAD combined with CBL was adopted in the experimental group while the traditional teaching method was adopted in the control group. The final examination and questionnaire survey were used to evaluate the learning effect. T test and χ2 test were used for data analysis. Results In the final examination, the scores of the knowledge mastery questions, the ability application questions and the total score in the experimental group were (47.8±4.7), (35.8±2.6) and (83.7±6.3) respectively, which were higher than those of the control group (45.0±4.3), (30.0±3.0) and (75.1±7.1), and the differences were statistically significant (all P<0. 05). The results of the questionnaire showed that 28 students (87.5%) in the experimental group and 12 students (41.4%) in the control group thought that their active learning ability had been improved. Thirty students (93.8%) in the experimental group and 20 students (69.0%) in the control group were satisfied with the classroom. The differences between the two groups were statistically significant (all P<0.05). Conclusions PAD combined with CBL teaching method in pharmacology theory teaching is helpful for students to grasp and apply knowledge, to cultivate their active learning ability and to improve classroom satisfaction.

    Application of the integrated teaching model of theory and practice based on outcome-based education principles in clerkship teaching of digestive system

    Objective To explore the effect of the integrated teaching model of theory and practice based on outcome-based education (OBE) in clerkship teaching of digestive system. Methods A total of 134 5th-year undergraduate students major in clinical medicine from Binzhou Medical University and in clinical clerkship in Department of Gastroenterology and Hepatology of Binzhou Medical University Hospital were selected from November to December 2021. They were divided into two groups by class, with 64 students in the experimental group and 70 students in the control group. The experimental group adopted the integrated teaching model of theory and practice based on OBE principles, while the control group adopted classroom and clinical internship learning in learning digestive system. Both groups took a comprehensive examination before and after the class, and the experimental group was given a post-course questionnaire on teaching effects. T-test was used to analyze the data. Results At the end of the course, the progress of the experimental group was higher than that of the control group (86.18±8.64 vs. 75.00±6.92, P<0.05). Theoretical knowledge and clinical comprehensive scores of the experimental group were higher than those of control group (43.38±7.50 vs. 38.79±7.49, 42.79±6.60 vs. 36.21±7.78, P<0.05). The results of the questionnaire showed that students in the experimental group were satisfied with the instructional design, participation, and evaluation, with values of (3.30±0.55), (3.11±0.62), and (3.20±0.59). Conclusions The integrated teaching model of theory and practice based on OBE principles is conducive to the mastery of theoretical knowledge, the development of integrated clinical competencies, and active participation in learning among medical students. It has also been recognized by students for teaching design and assessment.

    The impact of virtual simulation experiment teaching on learning effect in medical students

    Objective To summarize the effectiveness of the application of virtual simulation experimental teaching in medical courses at home and abroad. Methods Meta-analysis method was used. The literature on the application of virtual simulation experimental teaching in medical curricula at home and abroad in the databases of CNKI, Wanfang, VIP, PubMed, EBSCO and Web of Science during 2000 to 2021 were searched, and their quality was evaluated and analyzed according to the Cochrane systematic evaluation method. Results A total of 53 papers were included, including 10 in English and 43 in Chinese, with a total of 12 329 study cases. The quality of the literatures were evaluated with B grades. The results of Meta-analysis showed that students who participated in the virtual simulation experiment had higher scores in both theoretical [standardized mean difference(SMD)=0.914, 95%CI=0.729-1.100] and operational skills tests(SMD=1.385,95%CI=1.172-1.598) than those in traditional teaching, with statistically significant differences(all P<0.05). The virtual simulation experimental teaching group showed satisfaction in teaching methods[relative risk(RR)=1.245, 95%CI=1.038-1.494)], self-learning ability(RR=1.430, 95%CI=1.168-1.750), etc. were superior to traditional teaching methods, with statistically significant differences (all P<0.05). Conclusions The use of virtual simulation experimental teaching in medical courses is effective and worthy of further promotion.

    The impact of international certification of medical education on resident competency-based training after graduation

    Objective To study the impact of international certification of medical education after graduation on the competency-based training of residents. Methods In September 2017 and November 2021, that is, two years before and after the Peking University First Hospital obtained the international certification of post-graduate medical education in 2019, a competency questionnaire survey was conducted for the guidance physicians responsible for the standardized training of residents in hospital, to study the impact of the certification process on the cognitive level of the guidance physicians' competency and the training results of residents. The data of rates were compared by Chi-square test. Results This study included 348 valid questionnaires in 2017 and 196 valid questionnaires in 2019. The results showed that after the certification, the rate of meeting the standards of the three roles of leader, health advocate and scholar who guide the competency indicators of doctors has been greatly improved, among which, the selection of leadership that can be demonstrated in clinical practice has increased from 265 (76.1%) to 183 (93.4%), the promotion of the health of patients, communities and people from 283 (81.3%) to 188 (95.9%), and the promotion of the development and dissemination of medical knowledge from 306 (87.9%) to 190 (96.9%), all P<0.05. In terms of the evaluation of resident training effect, the evaluation of the guidance physician on the three roles of resident medical experts, leaders and health advocates has been greatly improved. Among them, the selection and reasonable use of prevention and treatment measures has increased from 268 (77.0%) to 173 (88.3%), and the ability to demonstrate leadership in clinical practice has increased from 220 (63.2%) to 143 (73.0%), which can promote the health of patients, communities and people was from 246 (70.7%) to 162 (82.7%), all P<0.05. Conclusions The international certification of medical education after graduation is helpful to improve the teaching ability of doctors and the competency evaluation of residents, and promote the quality of standardized training of residents.

    Job satisfaction and job burnout of ophthalmology resident training students

    Objective To study the current situation and influencing factors of job satisfaction and job burnout of ophthalmology residents in training. Methods From June to July 2022, a questionnaire survey was conducted among the students from 11 national ophthalmology residential training bases. The MSQ scale was used to evaluate job satisfaction, and the MBI-GS scale was used to evaluate job burnout. The data were analyzed by Spearman correlation analysis and multiple linear regression. Results A total of 177 participants participated in the study, and 52 (29.4%) were satisfied, 96 (54.2%) were not completely satisfied, and 29 (16.4%) were not satisfied. There were 27 people (15.3%) with no burnout, 113 people (63.8%) with moderate burnout and 37 people (20.9%) with severe burnout. It is found that job satisfaction is negatively correlated with job burnout (r=-0.657,P<0.001). Factors affecting overall job satisfaction were annual income (β=0.196), current highest degree (β=-0.125) and employment issues (β=0.062), all P<0.05, R2=0.222. The factors affecting the overall job burnout are the years of residential training (β=0.094) and employment issues (β=-0.060), all P<0.05, R2=0.129. Conclusions The ophthalmology residents has low job satisfaction and high job burnout. Income, education degree, the years of residential training and employment issues will affect their satisfaction and job burnout. Improving salaries and reducing anxiety about future employment, such as joining courses related to medical humanities, may improve job satisfaction and reduce job burnout.

    The application of the smart management system for educational tutors in residency standardized training

    Objective To evaluate the application of the smart management system of educational tutors in residency standardized training (RST). Methods A questionnaire survey method was used from November 2019 to August 2022, a total of 229 RST educational tutors(72), RST managers(65), residents(92) in Shanghai East Hospital, Tongji University were selected as the survey subjects, and a self-designed questionnaire was used to investigate their use of the educational tutor's smart management system (management system for short). The data were analyzed by chi-square test and analysis of variance test. Results The level of the management system by RST tutors, RST managers and residents in ease of use was [70(97.2%) vs. 63(96.9%) vs. 90(97.8%), P=0.398], and satisfaction was [72 (100.0%) vs. 65 (100.0%) vs. 92 (100.0%), P=0.453], the differences were not statistically significant. In the evaluation of the overall advantages of the management system by RST tutors, RST managers and residents, paperless management can save cost [(91.24±12.89) vs. (90.98±12.75) vs. (87.92±16.61), P=0.263], clear processes for full management [(88.14±12.65) vs. (87.80±13.16) vs. (84.15±17.16), P=0.158], simplicity of operation improving management efficiency [(88.49±12.67) vs. (86.20±13.11) vs. (84.61±16.93) P=0.245], and automatic matching of teacher and student satisfaction [(87.13±12.24) vs. (87.06±11.69) vs. (88.04±16.52), P=0.881], the differences were not statistically significant. After the establishment of the management system, the 2022 residents (81) completed the comprehensive examination pass rate [78 (96.3%) vs. 57 (87.7%), P=0.049] and the department examination results [(91.58±5.72) vs. (90.90 ±10.60), P=0.045] were higher than those in 2019 (65) before the establishment of the management system. In the annual assessment from August 2021 to August 2022, 71 (98.6%) tutors completed monthly interviews with residents. Conclusions The management system is highly appreciated and unanimously recognized by users, the system has certain advantages in terms of cost savings, management efficiency improving and automatically matching teachers and students. The management system is helpful to improve the qualification rate of the comprehensive examination of the residents and the examination results after graduation at Shanghai East Hospital, and to improve the efficiency of RST management of residential training and the quality of training.

    Application of multidisciplinary integrated teaching in cancer pain management training for medical staff

    The multidisciplinary integrated teaching aims to improve the ability to solve complex clinical problems, and focuses on the cultivation of trainees' critical thinking, team awareness and interpersonal communication skills. It is widely used in medical training. Taking the cancer pain management training of 96 medical staff in Chongqing as an example, this study describes the implementation of multidisciplinary integrated teaching. The teaching effect was evaluated through the cancer pain management cognitive level test and the cancer pain management practice survey. The paired t-test was used for the cognitive score of cancer pain management of the trainees before and after the training. The results showed that the total scores of the questionnaire of the trainees before and after the training were (2.67±0.79) and (2.76±0.89) respectively, and the difference was statistically significant (t=-2.44, P=0.015). Six months after the training, 80(92.0%), 50(57.5%), 32(36.8%) and 28(32.2%) of the trainees respectively participated in the work related to cancer pain management, joined the multidisciplinary team of cancer pain management, undertook cancer pain consultation or outpatient service, undertook cancer pain management training or teaching. Multidisciplinary integrated teaching can improve trainees' cognition of cancer pain management, help trainees to carry out clinical practice of cancer pain management, and is widely recognized by trainees.

    Development of the entrustable professional activities for operating room new entrant nurses

    Objective To develop entrustable professional activities of newly recruited nurses in the operating room. Methods From September to December 2021, the modified Delphi method was used to conduct two rounds of consultations with 21 experts from 8 provinces and cities including Beijing, Guangxi, Henan, Shaanxi, Heilongjiang, Shandong, Guangdong and Sichuan. Results The authoritative coefficient of experts reached 0.835; the positive coefficients of the two rounds of consulting experts were 91.3% and 100.0%, respectively, and the Kendall harmony coefficients were 0.206 and 0.288, respectively (P<0.05). A total of 11 entrustable professional activities and their content descriptions for newly recruited nurses in the operating room were constructed, and the expected confidence levels of each indicator at different time points were determined. Conclusions A high-quality entrustable professional activities for newly recruited nurses in the operating room and the expected confidence level at different time points are preliminarily developed, which can inform future competency-based training of newly recruited nurses in the operating room.

    The application of massive open online courses (MOOC) in higher medical education is becoming more and more widespread, the evaluation of medical MOOC has raised considerable concern. Thus the medical MOOC evaluation index system has constructed in 4 steps. First, the evaluation module of medical MOOC is determined based on literature research. Second, the evaluation elements of medical MOOC are extracted through case study. On this basis, the construction of index system is completed through Delphi method, and finally the index weights were calculated via yaahp software. The medical MOOC evaluation index system consists of 5 primary indicators and 13 secondary indicators and their weights as well as 26 points of concern. The construction of the medical MOOC evaluation index system is guided by experts through two rounds of consultation questionnaires, the Cronbach's alpha of which were 0.822 and 0.910 respectively, and the content validity coefficient were 0.974 and 0.923 respectively. The index system has a certain degree of reliability and validity and can provide some reference for medical schools to carry out medical MOOC construction and evaluation.

    Reliability and validity analysis of the Chinese version of the scale on community care perceptions among nursing students at vocational colleges

    Objective To analyze the reliability and validity of the Chinese version of the scale on community care perceptions(SCOPE) among nursing students at vocational colleges. Methods In March 2022, the Chinese version of SCOPE was used to investigate 609 nursing students at 2 vocational colleges in Shandong Province, and the reliability and validity of the scale were analyzed by internal consistency test, content validity index and factor analysis. Results The Chinese version of SCOPE includes a total of 31 items: effective component scale (10 items), placement scale (5 items), profession scale (14 items) and 2 separate items. The Cronbach's α coefficients of the total scale and 3 subscales were 0.963、0.954、0.894 and 0.937 respectively; the content validity index was 0.903; 4 common factors were extracted by exploratory factor analysis, the cumulative variance contribution rate were 71.373%、70.398%、65.998%. Conclusions The Chinese version of SCOPE has good reliability and validity, and it can be used to evaluate the perception attitude of community care among nursing students at vocational colleges.
