To understand the development in order history and current situation of international entrustable professional activities(EPAs) research. In 2020, in order to trace the general status of EPAs research, HistCite was employed to explore the number of papers, author distribution and research focuses based on the literatures about EPAs covering the period from January 2005 to September 2020 selected from Web of Science core collection. The results showed that the number of published papers in the field of EPAs appeared an overall upward trend. The United States has a big advantage in the number of owned papers, research institutions and journals. International EPAs research has developed rapidly recently, the areas which are in the forefront of medical discipline are also in the forefront of EPAs research. China is relatively weak in the field of EPAs research, so it is necessary to strengthen the capacity building for the research in this field.

    Abstract ( 68 )   PDF (873KB) ( 59 )
Objective To evaluate the effect of modularized simulation-based training mode in arthroscopic surgery. Methods Totally 111 refresher physicians who participated in the training of shoulder arthroscopic surgery simulation from April 2018 to October 2019 in Beijing Johnson & Johnson Training Center were recruited. All the participants conducted on-site training record and questionnaire survey. Non-parametric Mann-Whitney U test and T test were used to analyze the obtained data. Results On completion of the training, participants showed significant improvement in the time of grasping and transferring [(202.92±27.86) seconds vs. (123.44±18.93) seconds], for the time of suturing and knotting, the training also witnessed significant decrease [(333.77±24.36) seconds vs. (183.92±14.19) seconds] (all P<0.05). After the training, the times of grasping and transferring shortened [43.5(9.0,60.5)] seconds in tertiary hospital group and [40.0(19.5,147.0)] seconds in non-tertiary hospital group, while the time of suturing and knotting shortened [85.0(60.0,180.0)] seconds in tertiary hospital group and [120.0(60.0,260.0)] seconds in non-tertiary hospital group. There was no significant difference between the refresher physicians come from the tertiary hospitals and non-tertiary hospitals groups regarding the shortened time (all P>0.05). The orthopedics group improve [60.0(23.0,90.0)] seconds in the time of grasping and transferring and the sports medicine group improve [20.0(6.0,60.0)] seconds (P<0.05). For the time of suturing and knotting, [80.0(52.0,172.3)] seconds and [90.0(60.0,275.0)] seconds were improved respectively. The difference was not statistically significant (P>0.05). The refresher physicians satisfaction of the simulation training is 100.0% (111/111). Conclusions The basic surgical skills of refresher physicians from both orthopedics and sports medicine after the training of modular arthroscopic surgery simulation are significantly improved. The method is suitable for refresher physicians from hospitals of different levels and for both orthopedics and sports medicine.

    2020, 40 (12):  983-987.  DOI: 10.3760/cma.j.cn115259-20190712-00549
    Abstract ( 119 )   PDF (892KB) ( 108 )
Objective To explore the effect of simulated cases and small group case discussion teaching in a modular course of clinical thinking training for medical students in the internship stage. Methods From June 2018 to January 2019, 40 clinical medical students enrolled in 2014 in Tongji University School of Medicine were randomly divided into experimental group (n=20) and control group (n=20). The experimental group was given DxR teaching module first, then group discussion module, and the control group was taught in the reverse order. Students' evaluation of teaching methods was got by questionnaire. Score of DxR clinician and tests by multiple choice questions were used to assess the teaching effect. Results 76.7% (31/40) students thought that DxR module was helpful to cultivate clinical thinking ability, and 90.0% (36/40) students liked group discussion teaching and thought it was helpful to cultivate clinical thinking ability. The scores of mid-term examination and DxR score in the later stage of control group were significantly higher than those in the experimental group [(75.4±13.5) vs. (66.7±10.7), 57.3 (53.7,59.0) vs. 46.0 (43.0,53.0), all P<0.01]; the overall performance of DxR in the latter stage of the control group was significantly higher than that in the former stage [57.3 (53.7,59.0) vs. 46.0 (39.0,48.7), P<0.01]. Conclusions This modular course conducted by DxR clinician and small group discussion was implemented smoothly and were highly accepted by students. Small group discussion module ahead of DxR module could improve the teaching effect of DxR probably by forming friendly learning atmosphere and enhancing learning initiative. The application of two teaching methods could complement each other and improve clinical thinking ability from different dimensions.

    Abstract ( 248 )   PDF (1941KB) ( 137 )
Objective To analyze the research status, theme evolution and hotspots of the application of virtual simulation technology in medical education in China, so as to provide some ideas for further research and support application of virtual simulation technology in medical education. Methods Up to December 31st 2019, we retrieved the literatures from Wan fang Data, VIPs Database and China Knowledge Network (CNKI). Publishing trends, keywords' co-occurrence, keyword topic evolution trend and research hotspot distribution were examined by Cite Space and VOS viewer software based on the available literature data and visual graphs were then generated. Results Totally 682 literatures were retrieved, and the earliest study was reported in 1996. Knowledge structure indicates that “experimental teaching”, “virtual simulation experiment”and “virtual surgery” are the hot spots “virtual reality technology” and “virtual surgery” appeared early and have a good continuity of research hotspots. Research topics gradually diversified after 2013, such as “stomatology”, “rehabilitation medicine”, “plastic surgery” and other keywords showed up. Conclusions The research topics of virtual simulation technology in medical education of China are relatively broad. Since 1996, the overall growth trend of research literature is obvious. “virtual simulation experiment” is the current research hotspot. The research has experienced the initial stage, the expansion stage, and the outbreak stage.

    Abstract ( 43 )   PDF (871KB) ( 21 )
Objective To explore the training effects of Integration teaching model for extra-corporeal membrane oxygenation(ECMO). Methods In February 2020, 37 novice medical staff from the First Affiliated Hospital of Sun Yat-sen University were trained in ECMO course for 3 days in batches, and the teaching results were evaluated by the written test for the didactic course and the questionnaire for the practical course. Results All trainees passed the didactic course. After the 3-day training programme, 91.9% (34/37) of them thought that they invested more in active learning during the whole course; 59.5% (22/37) of them achieved a good level of technical evaluation. On the other hand, 67.6% (25/37) of the trainees have shown a good self-confidence in the priming course; however, 43.2% (16/27) of the trainees believe that they still need to strengthen their occupational competence in ECMO management. Conclusions The integration of traditional teaching and high fidelity simulation teaching can effectively improve the ECMO monitoring level and management skills of novice medical staff, but whether this training mode can be successfully transferred to the clinical field needs further research and confirmation.

    Abstract ( 58 )   PDF (890KB) ( 43 )
To discuss the different application of hybrid-problem based learning (hPBL) in pediatrics teaching between Shanghai Jiao Tong University of China and University of California Los Angeles of America, the pediatrics teaching in different educational system during four stages in China and America was analyzed. It is found that there are differences in the application of the hPBL teaching method, but each one plays its role. Problem-based learning(PBL) teaching method is widely used in medical education in both two medical schools. The university of California Los Angeles in America normalized the hPBL teaching method through pediatrics teaching practice lasted a long time, the learning outcomes are quite good. In Shanghai Jiao Tong University School of Medicine, the application of hPBL teaching method in pediatrics teaching practice lasts a short time, but also shows good learning outcomes.
