    Characterization of medical undergraduates in China

    Objective To investigate and analyze the characteristics of medical undergraduate students learning in different types of medical schools and different school systems in China. Methods Based on the data of China medical student survey in 2019, the characteristics of clinical medical undergraduates in China were statistically analyzed in three aspects as entrance, education process and outcomes, the differences were analyzed in two dimensions as college type and school system type. Results The results showed that 74.5% (7 497/10 062) undergraduates were enrolled matching the first choice as clinical medicine, 72.3% (7 271/10 062) of them had a clear career plan at that moment, and 86.9% (8 748/10 062) of them expressed their occupation willingness as a clinical doctor in the future. The ratio of urban vs. rural origin of newly enrolled students of the eight-year program, seven-year program, and five-year program were 78.9%(747/947)vs. 21.1%(200/947), 65.8%(1 042/1 583)vs. 34.2%(541/1 583) and 62.4%(4 698/7 532)vs. 37.6% (2 834/7 532)] respectively,the proportion of undergraduates with published papers [49.5%(469/947), 43.5%(689/1 583), 34.4%(2 589/7 532)] among the three programs were statistically significant (all P<0.01). Clinical undergraduates scored 3.89±0.80 points in regular learning behavior and 5.90±2.17 points in clinical capacity. Conclusions Compared with undergraduates of other disciplines in the country, the proportion of clinical medicine undergraduates enrolled by first choice is significantly higher, and the career development path is clearer. Students joining in the long-term curriculum have significant advantages in family background, research participation and paper publication. The active learning behavior of clinical medical students is relatively insufficient, and it is necessary to strengthen the capacity building in science and academic attitude, health and society and professionalism.

    How to encourage clinicians to join clinical teaching is an important issue in the new affiliated hospitals of college or university. Taking Beijing Tsinghua Changgeng Hospital Affiliated to Tsinghua University as an example, here we reported the development of a teaching incentive mechanism which merges teaching performance of clinicians into their payment package of routine salary. In this system, the payment to teaching performance is calculated delicately based on educator post and standardized teaching hours and then the education payment is merged into their routine salary counting. This system covering routine clinical performance and medical education simultaneously has been proved to be an effective encouraging and fair mechanism to pay clinicians who make contribution to clinical training of medical students individually as well as the member of a teaching team in the hospitals.

    Reflections on the reform and development of academic degree construction of integrated traditional Chinese medicine and Western medicine

    Integration of Traditional Chinese and Western Medicine play an important role in the prevention and control of COVID-19 Epidemic. By reviewing the history of 12 iterations of doctoral and master degree authorizations from 1981 to 2017, this paper analyses the development and changes of degree authorization of integrated traditional Chinese and Western medicine in China. From the perspective of medical education responding to the needs of social development, it is suggested that great importance should be attached to the construction of curriculum system and degree authorization in the field of integrated Traditional Chinese medicine and Western medicine disciplines. The efforts should be made to meet the national strategic needs, reform the examination system of degree authorization and strengthen discipline construction.

    Exploration on teaching reform of medical immunology course

    Consolidation of basic science and clinical medicine is a trend in medical education reformation. In the first semester of the 2019-2020 academic year, the teaching reform program of medical immunology was implemented among 2016 grade of eight-year curriculum undergraduates clinical medical students in Medical College of Sun Yat-sen University. The faculty from immunology department and their colleagues from surgery department in hospital were organized as a trans-sector team and implemented the education with cross-discipline performance. Examination and questionnaire survey were carried out after class to evaluate the learning outcomes. The results showed that 87.5%(70/80)of students were qualified, and 90.0% (72/80) of the students were interested in studying medical immunology. The teaching reform scheme of medical immunology course is beneficial to students' understanding and mastering of basic knowledge, which is highly praised by students and improves the teaching effect of medical immunology course.

    The correlation between perception of medical occupation from role modeling of healthcare providers and the development of professionalism by medical undergraduates

    Objective To study the perception of role modeling of clinical faculty during internship and professionalism development by medical undergraduates, and to explore the correlation between them. Methods Based on the data of perceived role models questionnaire for medical staff in hospital and professional identity scale of a medical college in Central China from china medical students Survey in 2019, the descriptive statistics, and Spearman correlation was used. Results The score of perceived medical workers' positive behaviors was [3.50(3.00,4.00)], and that of negative behaviors was [2.75(2.00,3.50)], the overall score of professional identity was(3.52±0.59). The overall score of professional identity was positively correlated with the score of perceived medical workers' positive behaviors (P<0.01) and had no statistical correlation with the score of perceived medical workers' negative behaviors (P=0.980). Conclusions The medical undergraduates from this college acknowledged that medical workers showed good role model behaviors, and their professional identity was on the upper-middle level. The positive role model behaviors from medical workers were positively correlated with professional identity of medical undergraduates.

    Analysis of attitude and influencing factors on online learning for clinical medicine undergraduates

    Objective To evaluate the online learning attitude and its influencing factors of clinical medicine undergraduates in Shanxi Medical University, and to provide a basis for improving the online education mode. Methods On February 2020, the survey data “online learning situation of Chinese clinical medical undergraduates” from the Shanxi Medical University was collected, and were analyzed by χ2 test and unordered multi classification logistic regression. Results Among 4 892 students, 88.5%(4 327/4 892)of them believed that online learning had obvious effect. The third grade students were 0.66 times more like the online teaching than the first grade students (OR=0.66, 95% CI=0.51~0.86). The students who were familiar with live teaching mode acknowledged that online teaching had obvious effect were 1.85 times (OR=1.85, 95% CI=1.23~2.78) of those who were not familiar with the on-line mode. The students who were familiar with MOOC were 1.74 times (OR=1.74, 95% CI=1.16~2.61) more likely to believe that online teaching had obvious effect than those who were not familiar with MOOC. For those who believed that online teaching would not increase learning burden, the possibility of recognizing that online teaching had a obvious effect were 2.57 times (OR=2.57, 95% CI=1.89~3.48) of those who believed that online learning would increase learning burden. The students who were very interested in learning believed that online teaching had obvious effect were 6.38 times (OR=6.38, 95% CI=3.67~11.11) of those who were not very interested. The students who very consciously choosed network resources for learning thought that online teaching had obvious effect were 23.09 times (OR=23.09, 95%CI=4.63~115.26) of those who did not choose network resources at all. Conclusions Clinical students had a positive attitude towards online teaching on the whole. Grade, previous online learning experience and learning characteristics of students were the influencing factors of online learning attitude.

    The standardized residency training of laboratory medicine is an important component in the training system of qualified laboratory practitioners, however, the training outcome is an emerging challenge. In this study, general-teaching, interactive-teaching and targeting-teaching were applied. Multi-dimensional evaluation and continuous feedback were conducted to accurately assess the effect of each teaching method. The application of this training mode for years is inspiring because of improved post competence of residence trainees.

    Application of conception-design-implement-operation model in emergency nurses' emergency first aid skills training

    Objective To explore the effect of conception-design-implement-operation(CDIO) mode in emergency nurses'first aid skills training. Methods All 70 nurses in emergency department were divided into observation group(35 nurses) and control group(35 nurses).The control group was trained by traditional micro-video teaching method, while the observation group was trained by CDIO mode.The training effects of the two groups were compared. Results The results of first aid knowledge assessment in the observation group were better than those in the control group [(86.46±5.27) vs. (77.57±5.60)], the comprehensive score of simulated based first aid practice in the observation group was better than that in the control group [(111.14±3.29) vs. (96.14±4.22)], the difference were all not statistically significant(all P<0.01). Conclusions The CDIO mode can effectively improve the initiative of training and learning in emergency nurses' first aid skills training process.The team cooperation,the use of first aid items and the comprehensive ability of communication in emergency nurses were also improved.

    Application of small private online course in clinical skill integration training

    Objective To explore the effect of small private online course (SPOC) in clinical skill integration training. Methods Experimental control method was used in this study. From September 2016 to June 2018, 248 five-year program students majoring in clinical medicine enrolled in 2013 and 2014 in Shanxi Medical University were selected as the research objects. The students in each grade were randomly divided into experimental group and the control group. The experimental group adopted the SPOC of clinical skills integration training mode, while the control group adopted the traditional single skill theory education combined with on-site demonstration training mode. The training outcomes of the two model groups were evaluated by the pass rate in the first stage of clinical fundamental skills stage I test of National Medical Licensing Examination and by the pass rate of the skill test of National Medical Licensing Examination and by the questionnaire survey results. Results The passing rates of the first stage I of National Medical Licensing Examination in the experimental groups of 2013 and 2014 [95.0% (57/60), 93.5% (58/62)] were significantly higher than those in the control group [83.9% (52/62), 81.3% (52/64)] (all P<0.05); the passing rate of the skill test of National Medical Licensing Examination in the experimental group of 2013 [98.3% (59/60)] was also significantly higher than that in the control group [87.1% (54/62)] (P< 0.05); the satisfaction rates of the students about their training mode in the experimental and control group were 90.6% (774/854) and 81.6% (720/882) respectively, and the differences were statistically significant (P<0.05). Conclusions The teaching model of clinical skill integration based on SPOC is helpful to improve students' clinical skill level and clinical thinking ability, which is recognized by students.

    Knowledge graph analysis of 3D printing model in clinical teaching at home and abroad

    Objective To review literatures about 3D printing model applied in clinical teaching based on the science mapping, so as to understand the literature characteristics, hotspots and progress to provide references for future research. Methods The relevant literatures were retrieved from the core collection of Web of Science and CNKI, Wangfang and VIPs and the science mapping was drawn by CiteSpace software and the analysis function of the CNKI web page. Results A total of 617 articles were reviewed, in which 395 were published in English and 302 in Chinese. The annual publication volume showed a gradual increasing in quantity. The journals with the highest volume of domestic and foreign journals were respectively China Continuing Medical Education and Chinese Medical Education Technology. In recent years, researches about 3D printing were focused on anatomy education, surgical simulation and orthopedic teaching. Conclusions The 3D printing model applied in clinical teaching had gradually attracted the attention of clinicians. However, the coverage of relevant research in China was limited. There are still some problems and limitations in the application of 3D printing model in clinical teaching.

    The application of laparoscopic virtual simulator combined with simple simulation box in the thoracoscopy training for resident doctors

    Objective To evaluate the application of laparoscopic virtual simulator combined with simple simulation box in the thoracoscopy training for resident doctors. Methods A randomized controlled trial design was used in this study. From October to December 2019, forty resident doctors of 2019 in Harbin Medical University Cancer Hospital who had not received any other form of endoscopic technical training were selected as the research objects,and they were randomly divided into experimental group and control group. The experimental group was trained with Lap Mentor laparoscopic virtual simulator combined with simple simulation box in skill teaching, and the control group was trained with simple simulation box. After the training, theoretical assessment, simulation operation evaluation, clinical skill, and questionnaire feedback were conducted in both groups. The data were analyzed by Fisher exact probability, Student's t-test and Wilcoxon Rank Sum Test for evaluation of teaching effect. Results In the simulated operation examination, the experimental group saved more time in the bean grasping training[115.5(21.5) vs. 92.0(38.0)], paper cutting training [225.0(19.3) vs. 180.0(22.0)] and continuous suture examination [483.0(24.0) vs. 370.0(25.0)] than the control group (all P<0.05); in the clinical practice examination, the OSATS scores of the experimental group were significantly higher than those of the control group (89.15±5.11 vs. 81.21±4.96; 88.10±4.42 vs. 77.05±5.12, all P<0.01). The questionnaire survey showed that the experimental group was better than the control group in improving learning interest[5.0(1.0) vs. 3.0(1.0)] and effectively improving skills [4.0(1.0) vs. 3.0(1.0)](all P<0.01). Conclusions Laparoscopic virtual simulator combined with simple simulation box is helpful to improve the training outcomes of endoscopic operation and other clinical skills with stimulation of learning interests.

    Reality hysteroscopy simulation based training system for residents' skill building in the field of gynecology and obstetrics

    Objective Based on reality hysteroscopy simulation training system, to explore the teaching effect of hysteroscopy system simulation training for standardized training trainees of gynecologic and obstetric residents. Methods Based on reality hysteroscopy simulation training system was established for 20 second-year residents in the Department of Obstetrics and Gynecology of the Third Affiliated Hospital of Guangzhou Medical University from January to December 2018. Residents will be asked to fill in a questionnaire for the training. Results One hundred percent(20/20) of the trainees thought that simulation training operation based on real hysteroscopy system was necessary for hysteroscopy skill training, the simulation training was helpful to improve the skills of hysteroscopy, and greatly improve learning interest and self-confidence. Conclusions The training mode with simulation of hysteroscopy system is suitable for the standardized training of gynecologic and obstetric residents in terms of skill building.

    Objective To investigate the effect of TeamSTEPPS combined with all factor real-time simulation of trauma treatment scene teaching in the standardized training of surgical residents compared with the traditional training method. Methods From January to December 2019, 120 trainees were randomly divided into the observation group and the control group with 5 in each team. The training was conducted by TeamSTEPPS combined with all factor real-time simulation of trauma treatment scene teaching and traditional teaching mode, and the effect of the training was evaluated by Team emergency assessment measure. Results The total score of the trainees in the observation group was significantly higher than that in the control group [the number of teams with total team score from 1 to 10: 0, 1, 1, 0, 3, 1, 2, 2, 1, and in the experimental group were 3, 2, 0, 3, 1, 1, 1, 1, 1, 1, 0, 0, 0], the difference was statistically significant(P<0.05). Conclusions TeamSTEPPS combined with all elements of real-time simulation of trauma treatment scene teaching can effectively improve the effect of standardized training of surgical residents in terms of team work spirit and clinical performance.

    Application of experiential teaching modules in anesthesiology internship for eight-year program students

    The eight-year program students from Peking University Health Science Center have only one week for their internship in anesthesiology. Since internship has a profound impact on students' choice of specialty. This article describes an internship training mode based on experiential learning theory supplemented by online teaching. It is a student-centered and task-oriented training method. And results show the mode improves students' satisfaction with anesthesiology practice and significantly stimulates students' interest in anesthesiology.

    “Team Strategies and Tools to Enhance Performance and Patient Safety”(TeamSTEPPS) is a team-work course which can integrate team cooperation skills with clinical practice, improve team performance in medical service, and improve medical quality and patient safety. In order to study the application effect of the course in anesthesia teaching, from December 2018, Peking Union Medical College Hospital used SimMan Team steps course was supported by 3G simulators. The simulated case training was carried out by interdisciplinary teams including anesthesiologists, surgeons and surgical nurses. The cognition and attitude of team cooperation of the trainees before and after the training were compared, and the effect of the course was evaluated. Up to December 2019, a total of 48 trainees have been trained, and each trainee has received two team training on simulated cases. All trainees are satisfied with the course. TeamSTEPPS course can enhance trainees' team cooperation cognition and attitude, and the willingness to provide help to improve the clinical performance of anesthesia team.

    Objective To explore the application effect of Mini-CEX in clinical probation teaching based on cases. Methods Experimental control method was used in this study. Sixty undergraduates of five-year program enrolled in 2015 in Harbin Medical University who was taking clinical probation from February in 2018 to January in 2019 were selected as the research objects. The students were randomly divided into control group and experimental group distributing with 30 in each. In clinical probation teaching, Mini-CEX evaluation method was used in the experimental group and traditional evaluation method was used in the control group. After the probation, comparing two groups for clinical diagnosis and treatment, and collect the comment from experimental group about students' satisfaction about Mini-CEX evaluation. Results The scores of medical history inquiry of experimental group and control group were (7.77 ± 0.82) and (7.10 ± 1.21), respectively; The physical examination scores were (7.40 ± 0.81) and (6.83 ± 0.99) respectively; The scores of humanistic care were (7.30 ± 1.12) and (6.07 ± 1.89) respectively; The clinical judgment scores were (7.47 ± 0.86) and (6.77 ± 1.48) respectively; The scores of communication skills were (7.77 ± 0.86) and (6.93 ± 1.53) respectively; The scores of organizational effectiveness were (7.20 ± 0.61) and (6.57 ± 1.10) respectively; The overall performance scores were (8.13 ± 0.68) and (7.40 ± 1.19) respectively; The differences were statistically significant (all P<0.05). The questionnaire results show that 93.3% (28/30) of the experimental group students are satisfied with the Mini-CEX method. Conclusions The application of Mini-CEX method in clinical probation can improve the quality of student probation.

    Exploration of applying “Deming cycle” in the training of supervisors of medical graduates

    This study is aiming for improving the capacity building of graduate supervisors of clinical medicine through application of “Deming cycle” in Peking University Third Hospital. The improvement of training outcome is focused on four issues as top-level design, training implementation, feedback and improvement. The study showed that the training system for graduates' supervisors based on Deming cycle is effective. Medical schools should establish a complete, standardized and scientific training system for the construction of supervisors' team by improving the management system, the overall quality of the training team, and strengthening the construction of the training connotation. Thus comprehensively improving the quality of training.

    Objective To investigate the current situation of research capacity and the related integrity knowledge in medical graduates. Methods From July to August 2019, 151 medical graduates from Peking University Third Hospital were investigated by using the research ability scale and the research integrity knowledge questionnaire. Results According to the standardized score of the hundred mark system, the total score of research capacity scale was [88(72,95)] and research integrity knowledge questionnaire was [84(77,88)].The scores of “statistical knowledge” and “research misconduct” were low[67(50,100), 81(77,88)]. Conclusions The research capacity and the realted integrity knowledge in medical graduates were fairly good. It is necessary to strengthen the training of “statistical knowledge” and the role of “practice”; improve the understanding of “Research misconduct” and create a good environment of research with cooperation of tutors, academic training and social support.

    The exploration of constructing a scientific assessment system for medical disciplines

    To reverse unscientific value orientation , it is urgent to construct a scientific assessment system of medical disciplines. Through reviewing the assessment history and the development trend of medical disciplines, researching and interviewing relevant experts, and combining the empirical study, it is tried to put forward thoughts and suggestions for the construction of medical discipline assessment index system, which covers a stratified and classified assessment indexes on strategy building of discipline development, an in-process evaluation index for talent bring up, the evaluation indexes for scientific research and nationwide needs-based application, evaluation indexes for sustainable faculty development, and evaluation system of medical disciplines in terms of academic reputation as well as social contribution.

    Evaluation of the performance appraisal system for resident standardized training teachers

    Objective To evaluate the application effect of the performance appraisal system of the resident standardized training teachers. Methods From January to December, 2019, based on the hospital resident training management information system of Shanghai Ninth People's Hospital, Shanghai Jiao Tong University School of Medicine, the performance appraisal results of 603 resident standardized training teachers were selected. The scores of the base directors, residents and management personnel were analyzed, and the comprehensive performance evaluation scores were calculated. Results The performance evaluation scores of the base director, resident and management department for the resident standardized training teachers are [90(85, 95)], [100(98,100)], [69.0(54.6, 79.4)]. The comprehensive performance evaluation score of teachers is [82.6(77.4,87.6)]. Conclusions The evaluation results are basically consistent with other literature reports. The performance evaluation index system of resident standardized training teachers has certain representativeness and objectivity, which can effectively evaluate the main work and teaching effect of resident standardized training teachers.

    The curriculum system on physicians' professionalism of Chiba University in Japan

    Japan gradually developed a curriculum system in physicians' professionalism training, whose successful experience may be valuable for China.This study summarized the goal and curriculum “Physicians' Professionalism” training from the “Model Core Curriculum for Medical Education”, launched by Japanese Ministry of Education, Culture, Sports, Science and Technology. Taking the Chiba University as an example, the authors introduces 4 modules of the professionalism curriculum, 36 items in 6 domains of competencies for graduates, and concludes the 6 characteristics of the medical professionalism curriculum, which is a potential reference for the curriculum development of medical professionalism in China. Moreover, the authors introduced and analyzed the characteristics and strengthen of the curriculum system on professionalism comprehensively, which may be a reference for the cultivation of physicians' professionalism in China.

    Hou Zonglian's ideology on medical education and the practice in Fujian

    Hou Zonglian is one of the founders of physiology in China and a famous medical educator in the history of medical education not only in Fujian but even also in China. This research reviewed journals, newspapers and other publications to collect a series of education concepts that Hou Zonglian put forward in medical education, scientific research and summarizes some education practices that Hou Zonglian carried out in Fujian focuses on system organization, discipline development and the talent team building. The research also discusses Hou Zonglian's great contribution to the development of medical education.
