临床医学专业学生全科医学科毕业实习的组织与评价

Graduation practice of general practice on clinical medicine major undergraduate in clinical teaching hospital

摘要: 目的 探索临床医学专业本科生全科医学科毕业实习的组织和评价。方法 以中国医科大学2011级七年制临床医学专业144名学生为研究对象。将学生随机分为实验组(72人)和对照组(72人)。实验组学生安排在中国医科大学附属第一医院全科医学科病房参加为期一周的实习,对照组学生安排在相同医院的其他内科学病房实习一周。采用出科考试、问卷调查和访谈方法评价教学效果。采用SPSS 11.5进行数据处理。结果 比较两组学生的出科考试成绩和对实习的满意度,其差异无统计学意义(P>0.05)。调查结果显示,实验组学生对全科医生制度、三级医院全科医学科、社区和大医院之间的双向转诊、全科医生连续性服务、全科医生综合性服务的认识的评分均高于对照组,其差异具有统计学意义(P<0.05)。实验组学生未来从事全科医学的可能性显著高于对照组,组间差异具有统计学意义(P<0.05)。结论 面向临床医学专业开展全科医学科毕业实习,完善了医学生全科医学教学体系,深化了医学生对全科医学的认识,增加了医学生未来从事全科医学的可能性,对全科医学的发展和全科医生制度的实施具有重要的现实意义。

Abstract: Objective To implement and evaluate clinical practice of general practice on clinical medicine specialty undergraduate in clinical teaching hospital.Methods 144 undergraduates of clinical medicine specialty in China medical university randomly divided into experimental group (72 undergraduates) and the control group (72 undergraduates). The experimental group took part in graduation practice in general practice department in the first affiliated hospital of China Medical University ward for a week, while the control group took part in graduation practice in other medicine department ward in the first affiliated hospital of China Medical University ward for a week. The examination, questionnaire and interviewing method were used to evaluate the teaching effectiveness. SPSS 11.5 was used for statistical analysis.Results The results of the experimental and satisfaction did not show statistically significant difference between the experimental group and the control group (P>0.05). In the result of questionnaire survey, the awareness of the experimental group about GP system, department of general practice in tertiary hospital, referral system between community and large hospital, sequential service of general practitioner and coordination service of general practitioner was better than that of the control group (P<0.05). The possibility of the experimental group in the future working in general practice is significantly higher than that of the control group (P<0.05).Conclusions Graduation practice of general practice in clinical teaching hospital perfected the teaching system in general practice, increased the awareness of general medicine and the possibility of the students in the future working in general practice, and is of practical significance to the development of general medicine and the establishment of the GP system.
