住院医师显微外科初级培训课程的教学实践探索

Exploration on teaching practice of primary training course of microsurgery for residents

摘要: 目的 探讨在住院医师中开设显微外科初级培训课程的教学效果和适用对象。方法 本研究采用资料分析方法。以2020年3月至2022年3月在北京大学人民医院接受规范化培训的39名住院医师为研究对象,根据既往研究构建显微外科初级培训课程,由多个学科临床医生进行显微外科初级培训课程授课,采用理论知识考核、操作技能考核和问卷调查评价课程教学效果及其适用对象,组间比较采用独立样本t检验和方差分析。结果 39名住院医师的理论考核成绩(14.5±2.8)分,操作技能考核成绩(64.0±3.4)分,总成绩(78.7±3.8)分,合格率100.0%(39/39),优秀率53.8%(21/39)。课程前后住院医师的显微外科操作自信程度评分差异具有统计学意义[(1.3±0.6 )分比 (2.9±0.9)分, P<0.001]。不同科室、学历、既往培训年限和既往有无技能培训和相关经验的住院医师成绩差异均无统计学意义[如培训年限≥3年和<3年的住院医师理论知识成绩:(14.7±2.7)分比(14.4±2.9)分, P=0.741;操作技能考核成绩:(64.2±4.2)分比(63.9±2.9)分, P=0.776]。住院医师还提出了增加实物练习、调整上课时间等建议。结论 显微外科初级培训课程教学效果良好,有助于在住院医师中科普显微外科知识,培训其显微外科技术,并提高其操作信心。显微外科初级培训课程可以在不同科室、学历、培训年限的住院医师中开展。显微外科初级培训课程仍需不断完善与改进。

Abstract: Objective To explore the teaching effect and applicability of microsurgery primary training course for residents. Methods The data analysis method was adopted. A total of 39 residents who received standardized training in Peking University People's Hospital from March 2020 to March 2022 were selected as the research subjects. According to previous research, the primary training course of microsurgery was constructed. The primary training course of microsurgery was taught by a multidisciplinary team of clinicians. Theoretical knowledge assessment, operational skills assessment and questionnaire survey were used to evaluate the teaching effect and its scope of application. Independent sample t-test and analysis of variance were used for comparison between groups. Results The theoretical and practical scores among all 39 residents were (14.5±2.8) and (64.0±3.4), respectively. The total score was (78.7±3.8). The qualified rate was 100.0% (39/39), and the excellent rate was 53.8% (21/39). There was a statistically significant in the degree of microsurgical operation confidence of residents before and after the course [(1.3±0.6) vs. (2.9±0.9), P<0.001]. There was no statistically significant in the scores of residents among different departments, education background, years of training, previous skill training and relevant experience [for example, the scores of theoretical knowledge of residents with ≥3 years of residency and < 3 years of residency were[(14.7±2.7) vs. (14.4±2.9), P=0.741]; the score of operation skills assessment was[(64.2±4.2) vs. (63.9±2.9), P=0.776]. Residents also put forward suggestions such as increasing physical exercises and adjusting class time. Conclusions The teaching effect of the primary training course of microsurgery is good, which is helpful to popularize the knowledge of microsurgery among residents, train their microsurgery skills, and improve their operation confidence. The primary training course of microsurgery can be carried out among residents in different departments, educational background and training years. The primary training course of microsurgery still needs to be improved.
