住院医师规范化培训全程导师指导能力培训项目的开发设计与实施

Design, development and implementation of mentorship and mentoring training program for the standardized residency training

摘要: 在住院医师规范化培训(简称住培)中,住培全程导师的指导能力对住院医师的培养至关重要。本研究团队在建立上海市东方医院住培导师制体系的基础上,采用经验萃取技术凝练出了住培全程导师所应具备的核心指导能力,并设计了导师指导能力培训项目,使其掌握在定期交流、随机指导、纠正偏误3大辅导场景中应具备的创建目标、“说、演、练”、正确反馈3项核心能力,并实施相应的9种关键行为。2020年11月至2021年12月共培训167名导师,课后问卷调查结果显示,导师对课程内容、授课讲师、设备环境、总体评价的满意度平均评分均为9分以上(满分10分),导师对导师制的理解体会在课前课后的评分差异具有统计学意义(8.6分比9.4分,P<0.001)。对住院医师的满意度调查结果显示,95%以上的住院医师对导师实施的关键行为表示非常满意和满意。培训项目得到了导师的认可,有助于提升导师的实际带教能力并得到了住院医师的认可。

Abstract: In the standardized resident training (resident training for short) , the guidance ability of the resident supervisor is very important to an effective training. With the support from a newly constructed system of mentorship in Shanghai East Hospital, we adopted the method of experience extraction to condense the core guiding ability that the full-course mentorship should possess, and designed the mentorship and mentoring training project which covered three core competencies of creating goals, “speaking, acting and practicing” and correct feedback in the three situations of regular communication, random guidance and correcting errors with nine corresponding behaviors. A total of 167 resident trainees joined the project from November 2020 to December 2021. The results of an after-school questionnaire showed that the instructors' satisfaction with the course content, the lecturers, the equipment facilities and the overall evaluation were all above 9 points (out of 10 points). There was a significant difference in the scores of tutor's understanding of tutorial system before and after class (8.6 vs. 9.4, P<0.001). The results of the satisfaction survey showed that over ninety-five percent of the trainees satisfied or very satisfied with the key behaviors demonstrated by their supervisors. The training program was approved by the trainers, since it, improve the trainers' teaching ability and also approved by the trainees as significant by their learning outcomes.
