新型冠状病毒肺炎疫情期间住院医师职业认同感和职业忠诚度状况研究

Study of the professional identity and professional loyalty of standardized training resident during COVID-19 epidemic

摘要: 目的 探讨新型冠状病毒肺炎(以下简称新冠肺炎)疫情期间规范化培训住院医师职业认同感和职业忠诚度状况,为采取针对性措施提供参考。方法 选取浙江省国家级住院医师规范化培训基地484名住院医师为研究对象。采用问卷调查方法,借助“我所从事的职业得到社会认可与尊重”和“我希望子女也成为医师”指标,分析住院医师职业认同感和职业忠诚度状况。采用参与抗疫和临床岗位、对抗击新冠肺炎疫情的态度、疫情期间所在地区等指标,分析住院医师职业认同感和职业忠诚度的相关因素。结果 71.3%(345/484)的住院医师认为医生职业获得了社会认可与尊重。46.3%(224/484)的住院医师不希望自己的子女成为医师。参加疫情一线工作(χ2=11.344,P=0.023)、对抗击疫情持积极态度(χ2=7.614,P=0.022)的住院医师的职业认同感较高,疫情期间主要在江苏、浙江、上海(以下简称江浙沪)地区工作的住院医师的职业忠诚度高于非江浙沪地区的住院医师(χ2=6.411,P=0.041),其差异均具有统计学意义。住院医师的职业认同感与职业忠诚度之间呈正相关(r=0.386,P<0.01)。结论 疫情期间住院医师的职业认同感较高,但职业忠诚度未有提高,参加防疫和临床工作、对抗击疫情的态度是职业认同感的相关因素,疫情期间所在地区和职业认同感是职业忠诚度的相关因素,因此要重点关注疫情一线、对抗击疫情持消极态度和非江浙沪地区住院医师的需求,培养住院医师职业认同感,从而提高其职业忠诚度。

Abstract: Objective To explore the status of professional identity and professional loyalty of standardized training resident during COVID-19 epidemic which provides a reference for development of strategy.Methods A questionnaire survey was conducted to 484 standardized training residents from two National standardized training base for resident doctors in Zhejiang. The survey was based on the indicators of “I gained social recognition and respect by engaging in this profession” and “I agree my children to engage in medical work” to analyze the status of residents' professional identity and professional loyalty. Using participation in anti-epidemic and clinical situations, attitudes to the COVID-19 epidemic, areas during the epidemic to analysis the relevant factors of professional identity and professional loyalty.Results On question of “I gained social recognition and respect by engaging in this profession”, the agreement rate of residents is 71.3% (345/484). On question of “I agree my children to engage in medical work”, the disagreement rate of residents is 46.3% (224/484). Residents who were on the front line of the battle against the epidemic (χ2=11.344, P=0.023) and who take a positive attitude towards the epidemic (χ2=7.614, P=0.022), have a higher professional identity. During the epidemic, the professional loyalty of residents who in Jiangsu, Zhejiang and Shanghai was higher than that in non-Jiangsu, Zhejiang and Shanghai (χ2=6.411, P=0.041). There is a positive correlation between professional training and professional loyalty of residents (r=0.386, P<0.01).Conclusions During the epidemic, the residents' professional identity was high, but professional loyalty was not raised. Participation in the battle against the epidemic, attitude towards the epidemic are relevant factors of professional identity. Areas during the epidemic and professional identity are relevant factors of professional loyalty. Priority needs to be given to the needs of residents who were on the front line of the battle against the epidemic, who were negative about the epidemic and who were in non-Jiangsu, Zhejiang and Shanghai. It is need to cultivate resident physicians' professional identity, thereby improving professional loyalty.
