住院医师规范化培训学员对缓和医疗认知的调查

A preliminary investigation of trainees' basic knowledge on palliative medicine in resident standardized training

摘要: 目的 初步了解住院医师规范化培训学员对缓和医疗基本理念的认知情况并提出改进建议。方法 采用自制调查问卷,通过“问卷星”在学员微信群里对2015年~2017年在南京医科大学第一附属医院参加住院医师规范化培训的263名学员进行调查,了解学员对缓和医疗基本理念的掌握情况和对推行缓和医疗的基本态度。结果 对缓和医疗基本理念欠缺的学员占比49.0%(129/263)。认同医务人员在缓和医疗中应起“权威建议”作用的学员仅占34.2%(90/263)。假设当处于生命终末期时,愿意主动为自己、亲属和患者实施缓和医疗的学员分别占比65.4%(172/263)、80.2%(211/263)和31.9%(84/263),差异具有统计学意义(χ2=133.240,P<0.05)。仅有28.1%(74/263)的学员认为我国目前推行缓和医疗时机成熟。对推行缓和医疗有信心的学员仅占比50.2%(132/263)。结论 我国目前的医学教育体系中存在着缓和医疗教育的缺失, 缓和医疗教育在医学教育体系的实施亟待得到普及、强化和规范。

Abstract: Objective To get a preliminary understanding of trainees' knowledge on palliative medicine's basic concepts in resident standardized training (referred to as “training”) and propose suggestions for improvement.Methods Using self-made questionnaire, 263 residents' standardized training trainees were investigated and conducted data summary in the First Affiliated Hospital of Nanjing Medical University from 2015 to 2017 by “questionnaire star” in their wechat group.Through the investigation targeted at basic concepts of palliative medicine and the professional attitude.Results The residents of 49.0%(129/263) had a flawed understanding of the basic concepts of palliative medicine, and there was no significant difference between different age groups, different academic qualifications and different training time (χ2test with all P>0.100). Only 34.2%(90/263) agree that medical staff should play an “authoritative role” in palliative medicine. Assuming that they are in the end of lives, 65.4%(172/263)、80.2%(211/263) and 31.9%(84/263) of those are willing to take the initiative to implement palliative treatment for themselves, their families and patients, and the difference was significant (χ2=133.240,P<0.05). Only 28.1%(74/263) of the respondents think that the time is ripe for our country to promote palliative medicine and only 50.2%(132/263) are confident in the implementation of palliative care in our country.Conclusions There is a lack of palliative medicine education in the current medical education system now and this is one of the important reasons that affect the palliative medicine in mainland China. Therefore, the implementation of palliative medicine education in the medical education system needs to be popularized, strengthened and standardized.
