电子病历模式下以问题为基础学习与床边学习在神经科实习教学中的效果比较

摘要: 目的 比较电子病历(electronic medical record,EMR)模式下以问题为基础学习(PBL)和床边学习(BBL)教学方法在神经科实习教学中的效果。方法 选择2014年1月至2014年12月在广州医科大学附属第一医院神经内科实习的2010级临床医学专业68名学生为研究对象,将其随机分为实验组(35人)和对照组(33人),分别采用PBL和BBL教学方法施教。实习结束后,比较两组学生的临床思维和EMR书写成绩,以及反映实习医生人文素质的患者满意度评分。结果 实验组学生临床思维考核成绩(P=0.020)、病历书写成绩(P=0.001)和总成绩(P=0.001)均明显高于对照组学生;实验组学生的患者满意度也明显高于对照组学生(P=0.016)。结论 在EMR模式下,PBL教学方法对神经科实习学生临床综合素质的培养效果优于BBL教学方法,是一种值得推广的教学方法。

Abstract: Objective To compare the teaching effect of neurologic interns between the problem-based learning (PBL) and bedside-based learning (BBL) methods under the model of electronic medical record (EMR).Methods From January 2014 to December 2014, the neurologic interns in the department of neurology of the First Affiliated Hospital of Guangzhou Medical University were randomly assigned to experimental group (n=35)and control group (n=33). The PBL and BBL methods were respectively used to teach the interns in two groups. After the experiment, a comparison was performed between the two groups with respect to the scores of clinical thinking, EMR writing and the patients' satisfaction questionnaire, as reflects the humanistic quality of the interns.Results After the trial, the scores of clinical thinking (P= 0.020), EMR writing (P=0.001) and the total (P=0.001) in experimental group were respectively significantly higher than those in the control group; the sore of patients' satisfaction questionnaire was as well significantly higher in experimental group than those in control group (P=0.016).Conclusions PBL is more effective than BBL on the training of clinical comprehensive quality to neurologic interns under the model of EMR, deserving to be popularized.
