不同时期儿科住院医师对规范化培训的选择和质量评价比较

摘要: 目的 比较不同时期儿科住院医师对住院医师规范化培训(简称住培)选择和质量评价的差异,为促进儿科住培基地规范化发展提供参考。方法 本研究采用问卷调查方法。选择2015年至2020年期间进入中国医科大学附属盛京医院儿科住培基地的147名学员进行调查,其中2015年至2017年期间进行住培的70名学员为A组,2018年至2020年期间进行住培的77名学员为B组,并对调查数据进行统计分析。 结果 B组学员比A组学员在5个方面的选择比例均较高,选择硬件条件好的医院[87.0%(67/77)比57.7%(39/70)],选择临床病例讨论[87.0%(67/77)比67.1%(47/70)],选择以疾病诊治能力培养为主[89.6%(69/77)比68.6%(48/70)],希望能够有教师带教[67.5%(52/77)比 51.4%(36/70)],认为考核制度比较系统[92.2%(71/77)比64.3%(45/70)],且差异均具有统计学意义(均P<0.01)。结论 不同时期儿科住培学员对基地的硬件条件、教学培训方式、教师的依赖性的选择和考核制度的评价不同,住培基地应持续改善硬件条件、合理安排住培课程、加强师资队伍建设、完善住培人员考核制度和方法,促进儿科住培基地规范化发展。

Abstract: Objective To compare the differences in selection and quality evaluation by pediatric residents from different periods of training, so as to provide reference to promote the development of pediatrics training base.Methods A study was conducted by the questionnaire survey. Totally 147 trainees in Department of Pediatric Medicine of Shengjing Hospital,China Medical University from 2015 to 2020 were recruited and 70 trainees from 2015 to 2017 were included in group A,while 77 trainees from 2018-2020 were grouped as B.Results Group B preferred to choose hospitals with good conditions[87.0%(67/77)vs.57.7%(39/70)], clinical case discussion[87.0%(67/77)vs.67.1%(47/70)] and focus on disease diagnosis and treatment[89.6%(69/77)vs.68.6%(48/70)], more trainees hoped to have a trainer to supervise their learning[67.5%(52/77)vs. 51.4%(36/70)]and more systematical examination [92.2%(71/77)vs.64.3%(45/70)] than group A (all P<0.01).Conclusions The pediatric residents from different periods have different choices for the hardware conditions of the base, the methods of teaching and training,the dependence of teachers,and the evaluation of assessment system. The base should continue to improve the hardware conditions, standardize the training courses, strengthen the construction of the teaching staff, and improve the assessment system and methods, which willbe helpful to the development of the pediatric residents base.
