湖北省2所地市级医院住院医师规范化培训学员岗位胜任力的调查

Clinical competency of trainees attending standardized residency training at two municipal hospitals in Hubei

摘要: 目的 调查湖北省2所地市级医院规范化培训住院医师的岗位胜任力现状,为同类型住培基地工作提供数据支持。方法 本研究采用问卷调查方法。2019年11月至2019年12月选择湖北省2所地市级医院住培基地的380名学员为研究对象,采用问卷对其进行调查,采用t检验、方差分析对数据进行分析,以多重线性回归分析岗位胜任力总体水平的影响因素。结果 学员岗位胜任力总体水平自我评价评分为(3.82 ± 0.55)分,全科基地学员评分[(3.59±0.56)分]最低。评分最低的2项能力是学术研究能力[(3.28±0.83)分]和信息与管理能力[(3.41±0.59)分]。多重线性回归分析显示,培训第一年(b=-0.30)、培训第二年的学员(b=-0.23)的岗位胜任力评分均低于已经结业学员(均P<0.05);全科基地学员的岗位胜任力评分(b=-0.36)低于其他科学员(P<0.01)。结论 本研究虽然充分肯定了现行阶段住培工作的成效,但学员在学术研究等岗位胜任力方面存在薄弱项,特别是全科基地学员,同类型地市级医院住培基地应针对这些薄弱项加强培训。

Abstract: Objective To evaluate clinical competency of trainees who attend standardized residency training (SRT) in municipal hospitals, and to provide data support for the work of SRT base. Methods From November to December 2019, a questionnaire survey was conducted among 380 physicians who attend SRT from at two prefecture-level hospitals in Hubei Province. Multiple linear regression was used to analyze the factors affecting the results on clinical competency. Results The score of clinical competencies in all participating physicians was (3.82±0.55), and the score in general practitioners (GPs) (3.59±0.56) was the lowest. The items with two lowest scores were research (3.28±0.83) and information management (3.41±0.59). Multiple linear regression analyses showed that, for trainees in the first year (b=-0.30) and the second year (b=-0.23)], their post competency scores were lower than those of trainees who had completed the training (all P<0.05). The competency score of GPs (b=-0.36) is lower than that of others (P<0.01). Conclusions Although this survey confirmed the effectiveness of SRT, weaknesses of clinical competency (e.g. academic research) were found in the trainees at municipal hospitals, particular for GPs. It is necessary to strengthen training specifically to fill the gap.
