八年制临床医学专业学生泌尿系统整合式课程见习满意度的混合式研究

A mix-methods study on the satisfaction of eight-year program medical students with the urinary system clerkship in an organ-system-based integrated curriculum

摘要: 目的 探讨器官系统整合教学模式下学生对泌尿系统整合式课程见习的满意度和需求,为进一步优化见习教学提供参考。方法 本研究采用定量和定性混合式研究方法,于2023年7月以北京大学2020级八年制临床医学专业学生为研究对象,对学生泌尿系统整合式课程见习满意度进行问卷调查,采用非参数检验和方差分析进行统计分析。基于调查结果,使用目的抽样法选取访谈对象,围绕泌尿系统见习体会及需求开展焦点小组访谈,使用NVivo软件对访谈内容进行编码,采用主题框架分析法进行资料整理和主题提炼。结果 在完成电子问卷调查的142名学生中,泌尿系统见习满意度评分为(4.26±0.63)分。见习满意度评分方面,男女生[(4.38±0.59)分比(4.14±0.65)分]、不同知识掌握程度[熟练(4.45±0.61)分、一般(4.18±0.56)分、不熟练(3.79±0.70)分]和知识运用能力[熟练(4.44±0.58)分、一般(4.10±0.62)分、不熟练(4.09±0.68)分]的学生间评分差异均具有统计学意义(均P<0.05)。共举办3场焦点小组访谈,提炼出增加临床实践、保障见习同质化、强化理论知识、改善教学环境4个主题。结论 学生对泌尿系统整合式课程见习整体满意度高,但也存在一定的提升空间。基于学生需求,未来可以在合理规划课程、加强科室间协作、完善师资培养和改善教学环境等方面进一步优化见习教学。

Abstract: Objective This study was aimed to investigate the satisfaction and demands of undergraduate medical students in the urinary system clerkship in an organ-system-based integrated curriculum to inform quality improvement of medical education. Methods This study was conducted in July 2023 with a mix-methods research approach. The 2020 grade eight-year program clinical medical students in Peking University Health Science Center were selected as the research subjects. The questionnaire was developed to assess the students′ satisfaction with the urinary system clerkship, and the data were analyzed using non-parametric tests and one-way analysis of variance. Based on the questionnaire survey results, the purpose sampling method was used to select students for focus group interviews to investigate the experiences and demands of the students. The interview contents were recorded, transcribed, and coded by NVivo software. Thematic framework analysis was used for data sorting and theme extraction. Results Among the 142 students who completed the online questionnaire, the satisfaction score of the urinary system clerkship was (4.26±0.63). Significant differences were observed in satisfaction scores among students of different genders [ (4.38±0.59) vs. (4.14±0.65)], abilities of knowledge mastery [proficiency (4.45±0.61), general (4.18±0.56) and non-proficiency (3.79±0.70)], and knowledge application [proficiency (4.44±0.58), general (4.10±0.62) and non-proficiency (4.09±0.68), all P<0.05]. Three group interviews were conducted. Four themes were concluded from the analysis: exposure to real clinical settings, homogenization of clerkship, reinforcement of the theoretical knowledge, and improvement of the teaching environment. Conclusions Students showed a high overall satisfaction level with the urinary system clerkship, but there was still room for improvement. Students′ opinions and suggestions will be fully adopted to improve the teaching curriculum of clerkship continually in the following aspects, including enhancing the overall curriculum planning, departmental collaboration, strengthening faculty training, and facility construction to improve clerkship satisfaction and effectiveness further.
