标准化病人在普通外科学急腹症临床见习教学中的应用

The application of standardized patient during clinical probation in acute abdominal disease in general surgery

摘要: 目的 探讨标准化病人在普通外科学急腹症临床见习教学中的应用效果。方法 2019年6月至7月,选取厦门大学医学院2015级五年制临床医学专业133名学生为研究对象,采用随机数字表法将其分为试验组和对照组。试验组68名学生采用标准化病人教学方法;对照组65名学生采用传统教学方法。通过比较两组学生的临床技能考核成绩、人文关怀考核成绩和问卷调查结果进行教学效果评价。结果 试验组和对照组学生的问诊成绩[(85.43±4.72)分比(80.93±3.16)分]、体格检查成绩[(83.75±3.81)分比(75.49±6.38)分]、首次病程记录书写成绩[(87.10±4.92)分比(83.91±6.26)分]、人文关怀成绩[(88.62±5.13) 分比(70.29±3.69)分]比较,其差异均具有统计学意义(均P<0.01)。问卷调查结果显示,试验组和对照组学生在提高自主学习兴趣[95.6%(65/68)、81.5% (53/65)],培养医患沟通技巧[97.1%(66/68)、47.7% (31/65)],提高临床思维能力[92.7%(63/68)、72.3% (47/65)],提高临床技能水平[94.1%(64/68)、75.4% (49/65)]等方面比较,其差异均具有统计学意义(均P<0.05)。结论 将标准化病人引入普通外科学急腹症的临床见习教学,有助于提高学生的临床技能和人文关怀水平,提高临床见习效果。

Abstract: Objective To explore the application effect of standardized patients in clinical probation teaching of acute abdomen in general surgery.Methods From June to July 2019, 133 clinical medicine students enrolled in 2015 from Xiamen University Medical College were selected and randomly divided into experimental group and control group. The experimental group (68) adopted standardized patient teaching method, while the control group (65) adopted traditional teaching method.The teaching effectiveness of the two groups of students was evaluated by clinical skills assessment,humanistic care assessment and the two groups of students to do a questionnaire survey of the effect of teaching evaluation.Results The students in the experimental group and the control group scored [(85.43 ± 4.72) vs (80.93 ± 3.16)], physical examination scores [(83.75 ± 3.81) vs (75.49 ± 6.38)], first disease course writing scores [(87.10 ± 4.92) vs (83.91 ± 6.26)], humanistic care scores [(88.62±5.13) vs (70.29±3.69)],The differences between the two groups were statistically significant (all P<0.01). The results of questionnaire survey showed that there were significant differences between the experimental group and the control group in the improvement of autonomous learning interest[95.6%(65/68),81.5% (53/65)], the cultivation of communication skills [97.1%(66/68),47.7% (31/65)], the improvement of clinical reasoning [92.7%(63/68),72.3% (47/65)], and the improvement of clinical skills[94.1%(64/68),75.4% (49/65)] (all P<0.05).Conclusions The introduction of standardized patients into clinical probation training of acute abdomen illness management in general surgery is helpful to improve the clinical skills and humanistic care of students, and improve the effect of clinical probation.
