    Exploring the high-quality development path of Tibetan medicine higher education under the new situation

    After more than 30 years of development, Tibetan medicine higher education has made great progress and development in terms of training scale, training level and training type, but the traditional teaching mode, backward teaching concept and weak teachers restrict the high-quality development of Tibetan medicine higher education. Based on the analysis of the current situation and causes of Tibetan medicine higher education, this paper proposes corresponding solutions: implementing the new development concept, consolidating the information construction of education, optimizing the layout of disciplines, increasing the construction of teachers, deepening the teaching reform, and trys to explore the path of high-quality development in line with the actual situation of Tibetan medicine higher education.

    Research and implications on formation mechanism and educational strategies of physicians′ professional behavior through the lens of the theory of planned behavior

    Physician′s professional behavior, a significant focus of continuing medical education, greatly impacts the quality of healthcare and the health outcomes of patients. Exploring the subjective decision-making mechanisms of physicians′ professional behavior is key to understanding and predicting them. The theory of planned behavior is a theoretical framework commonly used and widely proven effective, which broadened the theoretical perspective for professional behavior research. Drawing from a systematic review of relevant research, the study analyzed the influencing factors and formation mechanisms of professional behaviors from three perspectives: attitudes toward professional behavior, subjective norms, and perceived behavioral control based on the theory of planned behavior. Findings indicated that physicians′ assessment of whether their professional behaviors can improve patients′ health, the social pressure from patients and their families, as well as physicians′ knowledge and skills and control over the practice process exerted significant influence on their professional behaviors. Based on the above three perspectives, the study summarized the educational strategies for professional behaviors, aiming to provide theoretical and practical basis for future research on the continuing medical education of physicians′ professional behavior.

    Identification of key issues in the development of integrated traditional Chinese medicine and Western medicine discipline

    Objective To identify and analyze the key issues in the development of integrated traditional Chinese medicine and Western medicine discipline in China, and clarify the current bottlenecks in its development. Methods With the theme word ″integrated traditional Chinese medicine and Western medicine + discipline development/discipline construction/teaching″, literatures before March 1, 2023 were retrieved in databases such as CNKI, Wanfang Data and VIP Database for Chinese Technical Periodicals. The problem list was established through literature review and expert consultation, and key issues were identified by calculating the index of importance (Ii), seriousness (Si), and priority (Ri). Results A total of 43 references were retrieved, from which 14 main problems in the development ofintegrated traditional Chinese medicine and Western medicine discipline in China were summarized. Six experts in the field ofintegrated traditional Chinese medicine and Western medicine from 5 institutions participated in the consultation, the results showed that the top three issues in the index of priority were missing standardized training standards for resident physicians in integrated traditional Chinese medicine and Western medicine, immature teaching models for integrated traditional Chinese medicine and Western medicine, and insufficient clinical practice teaching bases for integrated traditional Chinese medicine and Western medicine. The index of priority was 0.151 8, 0.118 1 and 0.110 4, respectively. Conclusions There are various problems in the development of the integrated traditional Chinese medicine and Western medicine discipline, and the appropriate standardized training for resident physicians, teaching mode, and clinical practice base are the key issues. It is recommended to improve standardized training standards, optimize teaching modes, and accelerate the construction of clinical practice bases to support the development of integrated traditional Chinese medicine and Western medicine disciplines.

    Investigation on the current situation of sports literacy among undergraduate students at a medical college in Beijing

    Objective To analyze the current situation of sports literacy among undergraduate medical students (hereinafter referred to as medical undergraduates), to enhance their sports literacy, and to provide references for improvement measures. Methods In March 2023, 98 undergraduate students from a medical college in Beijing were randomly selected as the research subjects. A self-designed sports literacy questionnaire was used to investigate their sports participation awareness,knowledge and skills. Independent sample t-tests were used to analyze the differences in sports participation awareness, knowledge, and skills between students from different grades and genders. Results The scores for students′ awareness of sports participation, knowledge and skills were (8.50 ± 3.44) points and (7.69 ± 2.54) points, respectively. Female students scored lower in sports participation awareness (6.91 ± 2.70) and knowledge and skills (6.53 ± 2.83) compared to male students, who scored (9.85 ± 3.44) in awareness and (8.40 ± 2.12) in knowledge and skills. Third/fourth-year students scored (7.11 ± 2.57) in sports participation awareness and (6.86 ± 2.53) in knowledge and skills, which were lower than the scores of first/second-year students (9.63 ± 3.66) in sports participation awareness and (8.37 ± 2.37) in knowledge and skills. These differences were statistically significant (all P<0.05). Conclusions The scores for students′ awareness of sports participation, knowledge and skills are relatively low, especially among female and third/fourth year students. The promotion of sports literacy for female and third/fourth year students may improve the overall level of sports literacy of undergraduate medical students.

    Visual analysis of the application researches of artificial intelligence in the field of medical education based on Web of Science

    Objective To explore the basic situation and research hotspots of the artificial intelligence application in the field of medical education. Methods Based on Web of Science, the applied researches of artificial intelligence in medical education were retrieved during January 1986 to June 2023, and by the bibliometric methods to analyze the literature statistically and create a co-occurrence network of topic words, then drawing a topic clustering map and a cooperation network map between countries which the papers belong to. Results A total of 988 papers were retrieved, and the number of publications had significantly increased since 2017. The United States ranked first in terms of publication volume, with a total of 386 published papers accounting for 39.1%, while China had published 134 papers accounting for 13.6%. The high-frequency topic words and their frequencies were 152 times in machine learning, 62 times in deep learning, 32 times in natural language processing and 18 times in virtual reality, respectively. Conclusions The researches of the artificial intelligence application in the field of medical education are constantly increasing, and China need to strengthen publication volume. Machine learning, deep learning, natural language processing and virtual reality are the main research hotspots of the artificial intelligence application in the field of medical education currently.

    Application of scenario simulated teaching during clinical practice in dermatology and venereology

    Objective To explore the application effect of scenario simulation teaching in practice teaching of dermatology and venereology. Methods From August 2022 to September 2023, 154 undergraduate students majoring in five-year clinical medicine enrolled in 2019 of Xiangya Medical College of Central South University were selected as the research subjects. Scenario simulation teaching was used to carry out practical teaching of dermatology and venereology, professional assessment and surveys were adopted. Results Among 154 students, the theoretical knowledge score was (1.21±0.69) points(full score 3 points), and 47(30.1%) students rated as excellent; the diagnostic and treatment operation score was(2.24±1.01) points(full score 4 points), there were 104(67.5%) students rated as excellent. The number of students interested in the four stages of diagnosis and treatment drama, plenary discussion, organize answering questions, and theoretical explanation were 136(88.3%), 105(68.2%), 67(43.5%) and 34(22.1%), respectively. Conclusions Scenario simulation teaching can help strengthen students′ mastery of professional knowledge and improve their diagnosis and treatment abilities in practical teaching of dermatology and venereology.

    Application of a case database of 3D virtual simulation patients with typical periodontal diseases in pre-clinical periodontal education

    Objective To explore the application of a case database of 3D virtual simulation patients with typical periodontal diseases in pre-clinical periodontal education. Methods Thirty stomatological students from Peking University, enrolled in 2018, who were about to enter the clinical practice of periodontology, were recruited as the research objects. And the students were randomly divided into the control group and the study group, 15 in each group. The study group received training using the case database of 3D virtual simulation patients with typical periodontal diseases, while the control group received traditional teaching. After the training, the two groups were assessed on their clinical skills using the case database, and were scored by the system. The students were asked to fill in the feedback questionnaire to evaluate the case database. The t-test was used to compare the differences between the two groups. Results The mean score of history collection of the study group [(52.67±16.24) points] was higher than the control group [(36.80±10.84) points], and the difference was statistically significant (t=3.15, P=0.004). The results of the questionnaire show that all students fully agree or agree that this case library can enhance the ability and confidence of periodontal clinical reception. Conclusions The case database of 3D virtual simulation patients with typical periodontal diseases can improve the periodontal clinical consultation ability of stomatological students, and its efficacy was also widely recognized by students. The case database is expected to be further popularized and applied in the pre-clinical education.

    Analysis of the current status and development trend of clinical thinking research based on web of science

    Objective To explore the current status and development trend of clinical thinking research based on Web of Science (WOS). Methods Using the HistCite software, the literature from the WOS database established up to December 31, 2022 is taken as the research object. ″Clinical thinking″ or ″clinical reasoning″ are used as search keywords to analyze the number of related articles in the database, institutional distribution, research directions, and frontier topics, in order to outline the overall trend of research on clinical thinking. Results A total of 3,094 articles were retrieved. The research on clinical thinking can be roughly divided into three development stages: an initial stage (1966 to 1990, with an annual publication volume in single digits), a rising stage (1991 to 2011, with an annual publication volume between 50 and 100), and a steep rise stage (2012 to the present, with 341 articles published in 2022). The institution with the highest number of publications is McGill University in Canada, with the number of 80. High-frequency keywords include teaching, concordance, and script. The forefront topics of research are focused on the nature of clinical thinking, assessment of clinical thinking, and how to conduct clinical thinking teaching, with calls for the formulation of universal guidelines. Conclusions The volume of publications on clinical thinking research continues to grow. China has a low publication volume, with most publications coming from institutions in the United Kingdom and the United States. Research content mainly focuses on comprehensive analysis and reflection on clinical thinking, with insufficient empirical research. Specialized studies on scripts, teaching, and evaluation are gradually emerging. In the future, China should focus on theoretical research on clinical thinking, with emphasis on building teaching and evaluation systems for clinical thinking, as well as the application of emerging technologies in clinical thinking and the formulation of universal guidelines and standards. Additionally, efforts should be made to increase the international visibility and impact of research.

    Analysis of the effects of early exposure to clinical courses on core competency and early career planning of eight-year medical students

    Early exposure to clinical courses is an important introductory course for medical students. In December 2022, this study used 74 clinical eight-year medical students′ early clinical experience reflection texts as research data, and conducted qualitative text analysis using MAXQDA software. A framework was formed after three-stage coding, and teaching effectiveness analysis was conducted based on code frequency and coverage. Finally, an explanatory analysis model was formed based on learning objectives. Through various teaching methods such as verbal and nonverbal communication, simulated practice, and observation and communication, early exposure to clinical courses enhanced students′ cognition of core competency in patient care, communication and cooperation, professional ethics, knowledge and skills, and lifelong learning, and clarified career cognition and planning. This paper proposes an evaluation framework for the effectiveness of early exposure to clinical courses, a visualization of the logical relationship between learning objectives and student gains, and suggests improvements for the design of early clinical courses, including (1) optimizing course arrangements and balancing teaching methods, (2) emphasizing on teacher training and strengthening the integration of competency and clinical scenarios, and (3) promoting group study and enhancing the fullness of clinical experience.

    Objective This study was aimed to investigate the satisfaction and demands of undergraduate medical students in the urinary system clerkship in an organ-system-based integrated curriculum to inform quality improvement of medical education. Methods This study was conducted in July 2023 with a mix-methods research approach. The 2020 grade eight-year program clinical medical students in Peking University Health Science Center were selected as the research subjects. The questionnaire was developed to assess the students′ satisfaction with the urinary system clerkship, and the data were analyzed using non-parametric tests and one-way analysis of variance. Based on the questionnaire survey results, the purpose sampling method was used to select students for focus group interviews to investigate the experiences and demands of the students. The interview contents were recorded, transcribed, and coded by NVivo software. Thematic framework analysis was used for data sorting and theme extraction. Results Among the 142 students who completed the online questionnaire, the satisfaction score of the urinary system clerkship was (4.26±0.63). Significant differences were observed in satisfaction scores among students of different genders [ (4.38±0.59) vs. (4.14±0.65)], abilities of knowledge mastery [proficiency (4.45±0.61), general (4.18±0.56) and non-proficiency (3.79±0.70)], and knowledge application [proficiency (4.44±0.58), general (4.10±0.62) and non-proficiency (4.09±0.68), all P<0.05]. Three group interviews were conducted. Four themes were concluded from the analysis: exposure to real clinical settings, homogenization of clerkship, reinforcement of the theoretical knowledge, and improvement of the teaching environment. Conclusions Students showed a high overall satisfaction level with the urinary system clerkship, but there was still room for improvement. Students′ opinions and suggestions will be fully adopted to improve the teaching curriculum of clerkship continually in the following aspects, including enhancing the overall curriculum planning, departmental collaboration, strengthening faculty training, and facility construction to improve clerkship satisfaction and effectiveness further.

    Research hotspots and trend in scientific research training of medical graduates for professional degree in China

    Objective To discuss the hotspots and development trends in scientific research ability cultivation of medical graduates for professional degree in China. Methods Based on China National Knowledge Infrastructure, Wanfang Data, and VIP Database for Chinese Technical Periodicals, a search was conducted from the establishment of the database until May 11, 2023. Research literature on the cultivation of scientific research capabilities of medical graduates for professional degree in China was retrieved, and bibliometric analysis was conducted on the number of annual publications, distribution of publications, funding support, keywords, etc. The visualized knowledge maps were drawn by CiteSpace. Results This study included a total of 633 Chinese literature articles, published in 194 academic journals. The earliest research appeared in 2000, and the overall amount of literature published showed an upward trend. Of the 633 articles, 344(54.34%) received funding support. The research topic focused on research and clinical ability enhancement, mentorship, dual-track integration, training mode reform, and training quality. The recent keyword with the strongest citation bursts is “dual-track integration”. Conclusions The research on the cultivation of scientific research ability of graduate students majoring in clinical medicine in China has attracted more and more attention among scholars. The research hotpots are the topics of research and clinical ability enhancement, mentorship, dual-track integration, training mode reform, and training quality. Recently, the training mode of ″dual-track integration″ has generated an increasing amount of interests, which needs further exploration in the future.

    The application of an interest group approach to teach clinical research in standardized resident training

    The standardized residency training is centered on clinical skills and clinical thinking, but the demand for training the ability of clinical research for residents is becoming increasingly prominent. Traditional clinical research classroom teaching has problems such as boring content, outdated mode, and insufficient practice. In order to improve the clinical research practice ability of residents, the Fifth People′s Hospital of Shanghai, Fudan University launched clinical research interest group teaching in residency training in March 2022 to February 2023. This article introduced the teaching objectives, modules, forms, and organization of the interest group approach, and evaluated the teaching effectiveness through questionnaire surveys and clinical research design reviews. The evaluation results showed that all 22 residents participating in the clinical research interest group teaching were satisfied with the teaching activity and believed that the teaching would improve the clinical research practice and self-learning ability. The clinical research interest group teaching embodied the concept of student-centered teaching, and helped to improve the quality of residency cultivation.

    Perception of standardized residency training by trainees from the integrated traditional Chinese medicine and Western medicine specialty

    Objective To understand the perception of the standardized training among residents in integrated traditional Chinese medicine and Western medicine, aiming to provide references for the establishment of a specialized training program for residents in this field. Methods Custom questionnaire, based on relevant documents regarding standardized training, was administered to 74 residents undergoing training in four certified hospitals with integrated traditional Chinese medicine and Western medicine specialty in Shanghai in August 2022. Descriptive statistics were used for data analysis. Results A total of 71 residents participated in the survey. Among them, only 33.8% (24/71) believed that the current training program and standards for this specialty met their training needs; 67.6% (48/71) expressed their desire for improvements to the training program, advocating for more rational and targeted content and standards specific to this field, 62.0% (44/71) opted for training in tertiary hospitals specialized in this field; 88.7% (63/71) expressed their hope to enhance their clinical thinking skills in integrated traditional Chinese medicine and Western medicine through training. Conclusions The satisfaction level of residents in integrated traditional Chinese medicine and Western medicine with the current training program is not high. Residents investigated hope to establish a training program that meets the characteristics of talent cultivation in this specialty. Tertiary hospitals specialized in this field serve as the primary institutions for standardized training in this field, and the cultivation of clinical thinking skills ofintegrated traditional Chinese medicine and Western medicineshould be a major focus of the training program.

    Application of intensive training of critical bedside visual image technologies in critical care refresher physicians

    Objective To analyze the effect of intensive training of Critical Bedside Visual Image Technologies (TBVT) for critical care refresher physicians. Methods A controlled trial method was used. From January 2020 to May 2023, all refresher physicians in the department of Critical Care Medicine of Peking Union Medical College Hospital were studied. Among them, 54 refresher physicians between October 2021 and May 2023 were selected as experiment group and adopted TBVT for teaching, while the other 66 refresher physicians from January 2020 to September 2021 were selected as the control group, using traditional teaching methods. The effect was evaluated through the bedside imaging technology assessment results and the number of physicians qualified for independent duty by the two groups of refresher physicians. T-test, Mann-Whitney U test and chi-square test were used for data analysis. Results The qualification rates of pulmonary ultrasound, cardiac ultrasound, electrical impedance tomography, and sidestream dark-field technology in the study group were higher than those in the control group [100.0%(54/54) vs. 77.3%(51/66), 100.0%(54/54) vs. 71.2%(47/66), 98.1%(53/54) vs. 51.5%(34/66), 18.5%(10/54) vs. 3.0%(2/66)], and the differences were statistically significant (all P<0.05). The proportion of physicians in the study group qualified for independent duty was higher than that of the control group [25.9%(14/54) vs. 6.1%(4/66)], and the difference was statistically significant (P=0.004). Conclusions The TBVT can help critical care refresher physicians better master bedside visual image techniques and improve clinical abilities.

    Quality evaluation of medical psychology test paper based on item response theory

    Objective Applying item response theory (IRT) to analyze the quality of medical psychology course examination papers provides a reference for the quality of the evaluation of education and teaching. Methods In May 2022, we assessed the quality of 60 multiple-choice questions in the Medical Psychology test paper for 330 clinical medicine students in the class of 2016 at Shanxi Medical University. We tested the core assumptions of the IRT model and analyzed the model-data fit, as well as the item parameters of the test questions and the amount of information. Results The test data were consistent with the theoretical assumptions of IRT, and the model was well-fitted. The two-parameter logistic regression model was selected for parameter estimation. 59 (98.3%) of the test questions met the criteria for estimating the discrimination parameter, 54 (90.0%) of the test questions met the criteria for estimation of the difficulty parameter, 55 (91.7%) of the test questions were able to provide the minimum required amount of information(0.16), and 49 (81.7%) of the test questions were of satisfactory final quality, with the amount of information provided by the examination paper reaching 25. Conclusions The overall quality of the question papers was good, with the vast majority of the items being of high quality. However, it was necessary to modify or delete a small number of questions. This study can serve as a reference for the quantitative evaluation of examination papers in some university courses based on IRT.

    Revision and validation of professional identity scale for medical students

    Objective To revise and validate the a professional identity scale for medical students and conduct preliminary validation. Methods From November 2019 to May 2020, a scale item pool was formed based on social identity theory and previous research results on professional identity, combined with literature content analysis and project group discussion. The scale dimensions and items were added and subtraction through expert review to determine the scale of professional identity for medical students. In June 2020, a large sample of 5 807 medical students from 4 universities in Heilongjiang Province was tested by cluster sampling. Kronbach coefficient, split half reliability and factor analysis were used to test the reliability and validity of the scale. The present status of professional identity of medical students was analyzed. Results The professional identity scale of medical students was composed of 27 items in 6 dimensions, including professional cognition, professional emotions, professional commitment, professional behavior, professional expectations, and professional values. The total score of the scale is 162 points. Cronbach′s coefficients of each dimension were 0.81-0.94, the split reliability was 0.72-0.91. The cumulative variance contribution rate reached 82.32% after extracting six common factors by principal component analysis. The total score of professional identity of medical students was (134.24±21.53) points, which was at a medium high level, and the average score of professional behavior dimension was (29.02±5.31) points. Conclusions The professional identity scale of medical students has good reliability and validity, and can reasonably reflect the status quo of professional identity of medical students.

    Objective To develop a rubric to assess the ability of case summary of residents, and verify its validity. Methods In this study, the literatures combined with Delphi method were used to develop a rubric to assess the ability of case summary of residents.A total of 90 hospitalized cases of gastroenterology from the electronic medical record system in the First Affiliated Hospital of Anhui Medical University were selected by random number table method.The three raters scored according to the assessment rubric respectively. Cronbach coefficient was used to determine the internal consistency of five assessment criteria, the intraclass correlation coefficient (ICC) was used to determine the interrater agreement, and the Spearman rank correlation coefficient was used to determine the correlation among the five assessment criteria. Results After literature research and expert consultation, the assessment rubric of case summary ability was clarified,including 5 items of accuracy of clinical information, appropriate narrowing of differential diagnosis, translation of medical terms, use of semantic qualifiers, and global rating of clinical information summary.TheCronbach′s α coefficient of the second expert consultation questionnaire was 0.823 and the content validity index was 0.960.TheCronbach′s α coefficient of the assessment rubric was 0.801. The ICC of every item was 0.834, 0.690, 0.796, 0.802 and 0.760, respectively. The Spearman rank correlation coefficients for translation of medical terms and appropriate narrowing of differential diagnosis and global rating of clinical information summary are 0.71 and 0.69. Conclusions The assessment rubric of case summary ability of residents based on Delphi method has good reliability and validity. The assessment rubric has high internal consistency and inter-rater consistency.Its validation has been preliminarily verified in clinical practice.
