全病程管理应用于全科医师规范化培训眼科教学的研究

Use of the whole course management in ophthalmology standardized training of general practitioners

摘要: 目的 探索全科医师眼科规范化培训中采用全病程管理教学的可行性及学习效果评价。方法 2018年1月至12月在中日友好医院眼科进行全科医师规范化培训的全科医师共34人,参与全病程管理,完成后撰写病例报告,对全部34名全科医师及带教教师进行问卷调查,并对病例报告成绩做统计分析。结果 所有34人完成了学习过程,最终病例报告成绩为(86.1±8.2)分,不同性别[男性(84.9±9.9)分比女性(86.8±7.7)分,t=-0.67,P=0.507]、不同教育背景[西医(87.3±7.9)分比中医(85.5±8.4)分,t=0.594,P=0.557]、不同学历[本科(85.7±8.9)分比研究生(86.2±8.0)分,t=-1.63,P=0.871]、不同年级[第一年(83.3±9.5)分比第二年(86.0±7.4)分比第三年(91.8±4.0)分,F=2.404,P=0.107]的全科医师相比差异无统计学意义。问卷调查结果显示,全科医师认为教学安排合理[100.0%(34/34)],达到了学习目标[94.1%(32/34)],时间安排得当[100.0%(34/34)],通过全病程管理增强了医患沟通能力[100.0%(34/34)]、文献检索能力[97.1%(33/34)],加深了对疾病的认识[94.1%(32/34)],提高了学习兴趣[94.1%(32/34)],有助于培养临床思维[100.0%(34/34)]。结论 全病程管理教学应用于全科医师规范化培训眼科教学实用、可行,取得了预期的效果。

Abstract: Objective To explore the feasibility and learning outcome of the whole course management teaching method in ophthalmology training of general practitioners (GPs). Methods From January to December 2018,a total of 34 GPs were trained in the ophthalmology department of the China-Japan Friendship Hospital. They participated in the whole course management. At the end of the training a written case report was required and questionnaire for all 34 GPs and 8 teachers was conducted. Statistical analysis of the results was performed. Results All 34 GPs completed the learning process. The average score of the final case report was 86.1±8.2. The group differences divided by gender [(84.9±9.9) vs (86.8±7.7),t=-0.67, P=0.507], educational background [(87.3±7.9) vs (85.5±8.4), t=0.594, P=0.557], education level [(85.7±8.9) vs (86.2±8.0), t=-1.63, P=0.871] and grade [(83.3±9.5) vs (86.0±7.4) vs (91.8±4.0), F=2.404,P=0.107] were not statistically significant. The survey showed that most of the GPs believed that the teaching arrangement was professional[100%(34/34)],the learning target was achieved [94.1%(32/34)],time schedule was reasonable [100%(34/34)]. Most of them considered that the whole course management method helped them improve the ability to communicate with patients [100%(34/34)], literature retrieval ability [97.1%(33/34)], deepened the understanding of the diseases [94.1%(32/34)], increased the interest of learning [94.1%(32/34)] and helped to cultivate clinical reasoning [100%(34/34)]. Conclusions The application of whole course management in the standardized training of ophthalmology for GPs is practical and feasible, and has achieved the expected results, which is worth further promotion.
