摘要: 近20年来美国兴起的综合心胸外科一体化住院医师项目,以更早定向、一体化培训和专门的科研训练为特征,在保持高强度临床训练的同时,具有稳定的学术产出和人才吸引力,心血管外科住院医师培训一直沿用此项目。密歇根大学是美国历史最为悠久的研究型大学之一,也是实施该项目最有代表性的院校之一。本文采用资料分析和实地考察相结合的方法,从培养目标、培养路径、课程与轮转、教学方法与评价保障、临床胜任力与培养结果评价等方面对密歇根大学心血管外科专业人才培养模式进行阐释。借鉴其成功经验,我国需要结合国家医疗服务需求、现有制度、培养路径、培训供给侧约束等,逐步探寻适宜国情的心血管外科专业人才培训模式。

Abstract: Over the past two decades, the Integrated 6-year Cardiothoracic Surgical Residency Program (I-6) has emerged in the United States as a representative model characterized by early specialization, longitudinal training, and dedicated research development. By combining intensive clinical exposure with structured academic development, the program has maintained strong scholarly productivity and remained an established pathway for cardiovascular surgical residency training. The University of Michigan, one of the oldest research-intensive universities in the United States, is among the representative institutions implementing this model. Using document analysis and field observation, this article examines the cardiovascular surgery training model at the University of Michigan in terms of training objectives, pathway design, curriculum and rotations, teaching methods, assessment and quality assurance, clinical competence, and training outcomes.Its experience suggests that China should develop a context-appropriate training model for cardiovascular surgeons by aligning national healthcare needs, existing institutional frameworks, training pathways, and the constraints of training capacity.
