基于满意度视角的住院医师规范化培训现状调查分析

Investigation and analysis on current training status based on satisfaction of standardized residency training

摘要: 目的 调查北京大学直属附属医院和教学医院住院医师规范化培训(简称住培)现状和满意度,发现住培中存在的问题,提出改进意见。方法 本研究采用自行设计的电子问卷,于2023年7月对北京大学直属附属医院和教学医院在培住院医师和专业学位硕士研究生(统称为住培学员)进行调查,以描述性统计、方差分析、logistic回归和线性回归等方法分析培训满意度及其影响因素。结果 共回收467份有效问卷。住培学员的整体满意度为74.5%(348/467),对薪资[48.6%(227/467)]、福利[56.3%(263/467)]和培训强度[67.5%(315/467)]的满意度较低。专业学位研究生满意度低于住院医师[(3.50±1.16)分比(4.13±0.81)分,P<0.001]。约三分之二的住培学员每周工作时间超过50小时。培训管理、培训强度、培训指导的满意度和培训内容是否适合临床实际需要是整体满意度的影响因素(β值分别为0.435、0.311、0.157、0.094,均P<0.001)。结论 北京大学住培学员整体满意度尚可,临床工作强度和压力较大,应当进一步加强培训管理和培训指导,在薪酬待遇和培训强度等方面进行优化。

Abstract: Objective To investigate the training status and satisfaction of residents in affiliated hospitals and teaching hospitals of Peking University, identify the problems in standardized training for residents, and propose improvement suggestions. Methods This study used a self-designed electronic questionnaire to investigate the training satisfaction and its influencing factors of residents and professional master′s students in affiliated hospitals and teaching hospitals of Peking University in July 2023. Descriptive statistics, analysis of variance, logistic regression, and linear regression were used to analyze the training satisfaction and its influencing factors. Results A total of 467 valid questionnaires were collected as a result.The overall satisfaction rate of residents was 74.5%(348/467). The lowest satisfaction was observed with salary [48.6%(227/467)], benefits [56.3%(263/467)], and training intensity [67.5%(315/467)]. The satisfaction level of professional master's students was lower than that of general residents [(3.50±1.16) points vs. (4.13±0.81) points, P<0.001]. About two-thirds of residents work more than 50 hours per week. The overall satisfaction is influenced by factors such as training management, training intensity, satisfaction with training guidance, and whether the training content was suitable for clinical needs(β were 0.435, 0.311, 0.157, and 0.094, all P<0.001) . Conclusions The overall satisfaction of the residents in Peking University is acceptable, and the clinical work intensity and pressure are high. It is necessary to further strengthen training management and guidance, and optimize salary and training intensity.
