目的 探讨微信支持下的迷你临床演练评估( mini-clinical examination exercise,Mini-CEX)和操作技能直接观察评估(direct observation of procedural skills,DOPS)在肿瘤学临床教学考核中的应用效果。方法 制定Mini-CEX和DOPS评估表,于2018年3月开始,连续3个月对在汕头大学医学院附属肿瘤医院进行住院医师规范化培训的41名研究生和住院医师进行临床考核。带教教师借助微信填报问卷星格式的评估表,进行实时评分。统计分析评估指标的分数变化。结果 Mini-CEX的组织效能项目,第三个月评分(8.40±0.17)比第二个月评分(8.83±0.10)有所下降,其差异具有统计学意义(P=0.030)。DOPS中,执行临床操作的技术能力、与患者沟通的技巧等项目,第三个月评分[(8.87±0.17)和(9.13±0.20)]分别比第二个月评分有所提高分比[(8.17±0.20)和(8.11±0.28)],其差异均具有统计学意义(均P<0.01)。结论 在肿瘤学临床教学考核中,结合微信运用Mini-CEX和DOPS效果良好,具有操作简便、观察直接、时间地点的选择灵活等特点。

Objective To explore the effect of mini-clinical evaluation exercise (Mini-CEX) and direct observation of procedural skills (DOPS) evaluation supported by WeChat in assessing clinical capacity and clinical skills of medical students and residents. Methods Mini-CEX and DOPS evaluation forms in oncology were developed in this study. March to September 2018, questionnaire survey and WeChat Real-time Login were used to evaluate the performance of postgraduates and residents trained in the Cancer Hospital of Shantou University Medical College (SUMC). The evaluation and feedback were streamlined within 30 minutes and conducted for 3 consecutive months. Results In the Mini-CEX, organizational effectiveness significantly decreased in the third month compared with the second month [(8.40±0.17)vs(8.83±0.10), P=0.030].In the DOPS,the ability to perform operations and communication skills significantly increased in the third month compared with the second month with statistical difference [(8.87±0.17)vs(8.17±0.20), (9.13±0.20)vs(8.11±0.28), all P<0.01]. Conclusions In clinical teaching of Oncology, using Mini-CEX and DOPS evaluation supported by WeChat has the merits of easy operation, direct observation, flexible choice of time and place, as well as high reliability.
