实习医学生与住院医师规范化培训学员的叙事医学认知度和接受度比较分析

A comparative analysis of the awareness and acceptance of narrative medicine between medical interns and standardized residency training trainees

摘要: 目的 比较分析实习医学生与住院医师规范化培训(简称住培)学员对叙事医学(含平行病历)的认知度和接受度。方法 采用连续抽样法,于2024年1月至2025年7月在四川大学华西第二医院妇产科招募符合条件的调查对象,对62名实习医学生和118名住培学员进行叙事医学认知度问卷调查,并在叙事医学(含平行病历)理论知识培训后进行叙事医学接受度问卷调查。采用卡方检验和独立样本t检验分析数据。结果 实习医学生和住培学员的叙事医学认知度调查分别回收有效问卷62份和118份,住培组表示“未听说或不知道”叙事医学者占比多于本科组[50.8%(60/118)比24.2%(15/62),P<0.001];实习医学生和住培学员的叙事医学接受度调查分别回收有效问卷45份和98份,住培组对叙事医学激发工作热情、提高工作能力的积极作用接受度评分均高于本科组[(2.47±0.71)分比(2.20±0.73)分,P=0.038;(2.48±0.69)分比(2.22±0.67)分,P=0.039],愿意向他人推广者也多于本科组[89.8%(88/98)比73.3%(33/45),P=0.011]。结论 实习医学生对叙事医学(含平行病历)概念的认知度优于住培学员,而住培学员对叙事医学(含平行病历)的接受度更高。

Abstract: Objective To compare and analyze the awareness and acceptance of narrative medicine (including parallel charts) among undergraduate medical interns and residents undergoing standardized residency training. Methods A successive sampling method was adopted to recruit eligible participants in the Department of Obstetrics and Gynecology at West China Second University Hospital, Sichuan University, from January 2024 to July 2025. A total of 62 undergraduate interns and 118 residents undergoing standardized training were surveyed regarding their awareness of narrative medicine. Following a theoretical lecture on narrative medicine (including parallel charts), a subsequent survey was administered to assess their acceptance of narrative medicine. Data were analyzed using the chi-square test and the independent samples ttest. Results Sixty-two valid questionnaires from the intern group and 118 from the resident group were collected. Among all respondents, the proportion who reported having ″never known about″ narrative medicine was significantly higher in the resident group [50.8% (60/118) vs. 24.2% (15/62), P<0.001]. After the theoretical lecture, 45 valid questionnaires from the undergraduate intern group and 98 from the resident group were collected. The resident group demonstrated significantly higher acceptance scores regarding the positive role of narrative medicine in stimulating work enthusiasm and improving professional competence [(2.47±0.71) vs. (2.20±0.73), P=0.038; (2.48±0.69) vs. (2.22±0.67), P=0.039]. A significantly larger proportion of the resident group expressed willingness to recommend narrative medicine to others [89.8% (88/98) vs. 73.3% (33/45), P=0.011]. Conclusions The interns demonstrated a better awareness of the concepts of narrative medicine (including parallel charts) than residents, whereas residents showed higher acceptance of narrative medicine (including parallel charts).
