Objective To analyze the changes in medical students' perceptions of the doctor-patient relationship and willingness to engage in medical work before and after the COVID-19 outbreak, providing reference for medical educators. Methods Using a questionnaire survey method, a cluster sampling survey was conducted on medical students (405 before the COVID-19, 120 after the COVID-19) at a medical university in Heilongjiang Province before and after the outbreak of the COVID-19 (March to June 2019, November 2021 to April 2022). The propensity score matching method was used to balance the baseline characteristics of two groups of medical students (104 before and after the COVID-19). Using chi square test to analyze the differences in their perceptions of doctor-patient relationship and willingness to engage in medical work before and after the COVID-19. Results After using the propensity score matching method, there was no statistically significant difference in gender, grade, major, clinical internship experience, whether or not had witnessed or experienced hospital violence before and after the COVID-19[all P>0.05. for example, the number of female students before and after the COVID-19 was 68 (65.4%) and 66 (63.5%), P=0.772]. After the epidemic, the proportion of medical students who had a positive understanding of doctor-patient relationships increased compared with that before the epidemic [18 (17.3%) vs.5 (4.8%)], the willingness of medical students to work increased compared with that before the epidemic [102 (98.1%) vs. 89 (85.6%)], and the differences were statistically significant (all P<0.05). Conclusions After the outbreak of the COVID-19, medical students have shown a positive trend in their perceptions of doctor-patient relationships and their willingness to engage in medical work. It is recommended that medical educators guide and strengthen their understanding by actively exploring various educational methods according to the situation.
