Japan's Medical Licensing Examination regards the examinees' choice of question items with contraindication options as an important eligibility criterion. By analyzing the strategy and purpose of proposing question items with contraindication options of examinations from 2018 to 2020, this article finds that Japan's Medical Licensing Examination has brought the deeds of relative contraindications which may do harm to patients into the category of contraindications, thus expanding the connotation. Of the examination questions, the simulated clinical scenarios convey a sense of tension and preciseness to the examinees. Question items with contraindication options are randomly selected to urge examinees to remain vigilant at all times and develop their sense of respect for life. The above-mentioned features are worthy of consideration and reference. It is advisable that China's Medical Licensing Examination should be proposed to expand the connotation of the integrated question items of medicine and humanities, making the question items in line with clinical practice and constantly strengthening examinees' patient-centered concept.
