目的 探讨插管型喉罩在非麻醉学专业住院医师麻醉科轮转期间的教学应用与效果。方法 选择在首都医科大学附属北京友谊医院麻醉科进行轮转的非麻醉学专业30名住院医师为研究对象,进行面罩通气后直接喉镜下气管插管和置入插管型喉罩后经喉罩行气管插管的操作,观察插管的成功率,并对参与培训的住院医师进行教学满意度问卷调查。结果 30名住院医师均成功放置喉罩,28名住院医师经插管型喉罩成功进行了气管插管,成功率为 93.3%(28/30)。面罩通气后直接喉镜气管插管总成功率为63.3%(19/30)。2种气管插管操作成功率的差异具有统计学意义(χ2=14.0,P<0.05)。住院医师对插管型喉罩下气管插管操作的舒适度、熟练度、学习兴趣的评价高于直接喉镜下气管插管,差异均具有统计学意义(均P<0.05)。结论 在非麻醉学专业住院医师麻醉科的临床轮转中,插管型喉罩的引入有助于提高教学满意度和培训质量。
Objective To investigate the application of intubation laryngeal mask airway in the non-anesthesia residents during rotation of anesthesiology. Methods Thirty non-anesthesia residents in anesthesiology department were selected at Beijing Friendship Hospital of the Capital Medical University. They were required to perform the operation of bag-mask ventilation, laryngoscopic tracheal intubation, and insertion of intubating laryngeal mask airway and tracheal intubation through it. The success rates of insertion were recorded. Residents' perceptions were assessed on both methods. Results Thirty residents successfully placed the laryngeal mask for ventilation. Tracheal intubation along intubating laryngeal mask airway was successfully performed in 93.3%(28/30) patients as compared with 63.3% (19/30) in group of direct laryngoscopy (χ2=14.0, P<0.05). It showed that residents preferred intubating laryngeal mask airway more to traditional direct laryngoscopy for providing better learning skills with statistical differences (all P<0.05). Conclusions The use of intubating laryngeal mask can help non-anesthesia residents being trained with better intubation skills in the clinical rotation of anesthesiology.
