目的 总结志愿者模式在外科住院医师规范化培训临床技能结业考核中的应用效果。方法 回顾性分析复旦大学附属中山医院外科住院医师规范化培训5年来的相关数据,比较采用志愿者模式与采用传统模式进行外科住院医师规范化培训临床技能结业考核中,考核组织时间、考核费用、考核工作人员数量、考核总时间和平均时间等考核效率指标;考生及考官满意度以及志愿者学员第二年考核通过率等指标。结果 在外科住院医师规范化培训临床技能结业考核中,志愿者模式与传统模式相比,平均考核组织时间明显缩短(t=47.8,P=0.000),平均考核费用明显下降(t=36.1,P=0.000),平均考核时间明显缩短(t=20.8,P=0.000)、考核所需工作人员数量明显减少,考核效率明显提高。考生和考官的满意度明显提高(考生：χ2=18.6,P=0.000;考官：χ2=13.9,P=0.000)。总体平均考核通过率提高(χ2=3.8,P=0.031)。志愿者学员的平均出科理论考核通过率也比非志愿者学员有所提高(χ2=5.5,P=0.006)。结论 志愿者模式在外科住院医师临床技能结业考核中的应用可行,且比传统模式更有效率和优势。
Objective To summarize the application and effect of volunteers participation model in the surgical clinical-skills final achievement examination of the standardized medical residency training. Methods By retrospective analyzing the 5 years’ data of Zhongshan Hospital,Fudan University,compared the efficiency of the examination (preparation time,cost,staff numbers,whole and average exam time); The satisfactions of examinees and examiners; The pass rates of the volunteer trainees in the next year,before and after the application of volunteers participation model in the surgical clinical-skills final achievement examination of the standardized medical residency training. Results Compared to the traditional model,the application of volunteers participation model significantly reduced the average preparation time (t=47.8,P=0.000),average costs (t=36.1,P=0.000),whole & average exam time (whole：t=20.8,P=0.000,average：t=20.8,P=0.000) and staff numbers; improved the satisfactions of examinees and examiners (examinees：χ2=18.6,P=0.000,examiners：χ2=13.9,P=0.000); Increased the average pass rates (χ2=3.8,P=0.031) and besides,the pass rates of the volunteer trainees in the next year (χ2=5.5,P=0.006). Conclusions The application of volunteers participation model is practicable in the surgical clinical-skills final achievement examination of the standardized medical residency training,and has a more effective advantage of the traditional model.
