摘要

目的 探索医学影像控制系统在胸外科住院医师规范化培训(简称住培)中的应用效果。方法 选取2019年3月至2022年3月在哈尔滨医科大学附属肿瘤医院胸外科住培的100名学员为研究对象,按照随机数字表法将其分为试验组和对照组。试验组50名学员采用医学影像控制系统辅助教学,对照组50名学员采用传统教学方法。比较两组学员的出科成绩、临床技能考核和评价结果。采用t检验和χ2检验比较两组组间差异。结果 试验组学员出科考试的理论和阅片成绩均高于对照组学员,分别为(44.4±3.4)分比(38.8±3.9)分和(41.3±2.7)分比(34.3±2.3)分,其差异均具有统计学意义(均P<0.05)。试验组学员在影像诊断正确率[(84.4±7.4)分比(62.1±8.5)分]、影像鉴别诊断分析能力[(91.4±4.0)分比(74.5±7.1)分]和临床手术操作能力[(82.7±6.3)分比(75.2±6.7)分]考核得分均高于对照组学员,其差异均具有统计学意义(均P<0.05)。教师对试验组学员影像阅片能力的评价高于对照组学员,分别为(8.3±0.6)分比(7.4±1.1)分,其差异具有统计学意义(t=5.50,P=0.008)。结论 在胸外科住院医师规范化培训中应用医学影像控制系统可提高学员学习效果,增强其临床理论和实践能力,提高学员综合素质。

Abstract

Objective To explore the effect of medical image control system in the standardized training for thoracic surgery residents. Methods From March 2019 to March 2022, 100 residents who were undergoing standardized training in department of thoracic surgery, Harbin Medical University Cancer Hospital were selected as study subjects. They were randomly divided into study group and control group according to the random number table method. Fifty residents in the study group were taught with medical image control system, while fifty residents in the control group were taught with traditional method. The score of graduation examination, clinical skill assessment and evaluation feedback were compared between the two groups. T test and χ2 test were used to compare the differences between the two groups. Results The theory and image reading graduation examination scores in the study group were higher than those in the control group [(44.4±3.4) vs. (38.8±3.9),(41.3±2.7) vs.(34.3±2.3), all P<0.05]. The scores of the residents in the study group were higher than those in the control group in the accuracy of image diagnosis [(84.4±7.4) vs. (62.1±8.5)], the analysis ability of image differential diagnosis [(91.4±4.0) vs. (74.5±7.1)], and the ability of clinical operation [(82.7±6.3) vs. (75.2±6.7)](all P<0.05). The teacher's evaluation of the image reading ability of the residents in the study group was higher than that of the residents in the control group [(8.3±0.6) vs.(7.4±1.1),t=5.50, P=0.008]. Conclusions The application of medical image control system in the standardized training for thoracic surgery residents can improve the residents' learning effect, enhance their clinical theory and practical ability, and increase their comprehensive quality.
